頸椎單側(cè)關(guān)節(jié)突交鎖后椎間孔形態(tài)學(xué)變化

【摘要】: 目的:研究頸椎單側(cè)關(guān)節(jié)突交鎖后椎間孔上、下、內(nèi)、外各壁的壓力變化,以生物力學(xué)方法探討頸椎單側(cè)關(guān)節(jié)突交鎖后椎間孔形態(tài)學(xué)的變化 方法:對(duì)四具頸椎標(biāo)本,固定使C4-5為唯一運(yùn)動(dòng)節(jié)段,在椎間孔的內(nèi)側(cè)壁、外側(cè)壁、上壁及下壁四壁貼電極片,連接相應(yīng)測(cè)量?jī)x器以測(cè)量交鎖前后應(yīng)變。將標(biāo)本一端置于夾具中,另一端置于扳手中。向上述運(yùn)動(dòng)節(jié)段加載相應(yīng)剪切力(屈曲+旋轉(zhuǎn)),剪切力的大小由靜態(tài)電阻應(yīng)變儀反映出來(lái),測(cè)試出骨折脫位時(shí)的最大剪切力。同時(shí)測(cè)量旋轉(zhuǎn)角度并觀察其軟組織損傷情況。 結(jié)論:交鎖側(cè)及對(duì)側(cè)的外側(cè)壁及下壁均受到拉力,且被拉伸;內(nèi)側(cè)壁及上壁均受到壓力,且被壓縮。交鎖側(cè)外側(cè)壁所受的剪切力最大,且位移最大,其次為內(nèi)側(cè)壁。交鎖側(cè)外側(cè)壁所受的拉力遠(yuǎn)遠(yuǎn)大于對(duì)側(cè)外側(cè)壁的拉力,二者差別具統(tǒng)計(jì)學(xué)意義。內(nèi)側(cè)壁,上下壁的交鎖側(cè)與對(duì)側(cè)受力無(wú)顯著差別,無(wú)統(tǒng)計(jì)學(xué)意義。交鎖側(cè)各壁受力大小有量的差別,但無(wú)統(tǒng)計(jì)學(xué)意義。關(guān)節(jié)突分離所需要力為70N,旋轉(zhuǎn)角度為19.75°,棘上韌帶,棘間韌帶完全斷裂,關(guān)節(jié)囊破壞,前縱韌帶和后縱韌帶部分?jǐn)嗔?1/2以上纖維環(huán)破壞.
[Abstract]:Objective: to study the pressure changes of superior, inferior, inner and outer walls of intervertebral foramen after unilateral interarticular process interlocking. A biomechanical study on the morphological changes of intervertebral foramen after unilateral interarticular process interlocking of cervical vertebrae. In four cervical vertebrae specimens, C4-5 was fixed as the sole motor segment, and the medial wall and lateral wall of the intervertebral foramen were fixed. The upper and lower wall four-wall electrodes are attached to the corresponding measuring instruments to measure the pre-and post-lock strain. Place one end of the specimen in the fixture and the other end in the wrench. The shear force (buckling rotation) is loaded to the above motion segment. The shear force is reflected by the static resistance strainmeter and the maximum shear force is measured when the fracture is dislocated. At the same time, the rotation angle was measured and the soft tissue injury was observed. Conclusion: the lateral and inferior walls of the interlocking and contralateral sides are subjected to tension and tension, and the medial and superior walls are subjected to pressure and compression. The lateral wall of the interlocking side is subjected to the largest shear force and the largest displacement, followed by the inner lateral wall. The tension of the lateral wall of the interlocking side was much greater than that of the lateral wall of the contralateral side, and the difference was statistically significant. There was no significant difference between the interlocking side and the contralateral side of the inner wall, upper and lower wall, and there was no statistical significance. There was a significant difference in the strength of each wall of the interlocking side, but there was no statistical significance. The force required for the separation of the articular process was 70 N and the rotation angle was 19.75 擄. The supraspinal ligament, the interspinous ligament, the complete rupture of the interspinous ligament, the destruction of the joint capsule, the partial rupture of the anterior longitudinal ligament and the posterior longitudinal ligament, and the destruction of more than 1 / 2 of the fibrous ring were observed.
