腓骨后側(cè)入路治療踝關(guān)節(jié)骨折的臨床與解剖學(xué)研究

【摘要】: 目的: 1.探討腓骨后側(cè)入路單切口治療累及外踝和后踝的踝關(guān)節(jié)骨折的可行性、療效與優(yōu)勢。 2.測量腓骨遠(yuǎn)端三項解剖學(xué)指標(biāo),依據(jù)測量結(jié)果進(jìn)行腓骨遠(yuǎn)端后方鋼板的設(shè)計。 材料和方法: 1.隨訪我院2004年1月至2007年1月間踝關(guān)節(jié)骨折患者共計89例,按照Danis-Weber分型,Ⅰ型31例,Ⅱ型13例,Ⅲ型45例。按照腓骨骨折手術(shù)入路的不同分為兩組:A組常規(guī)腓骨外側(cè)入路植入鋼板,共計45例;B組應(yīng)用腓骨后側(cè)入路植入鋼板,共計44例。A組手術(shù)后常規(guī)即刻應(yīng)用短腿石膏托固定,B組術(shù)后不用任何外固定裝置,進(jìn)行踝關(guān)節(jié)功能康復(fù)訓(xùn)練,拆線后予以外固定直至臨床愈合拆除外固定。記錄術(shù)后外固定時間、傷口愈合情況、隨訪結(jié)束即取出內(nèi)固定后AOFAS評分。 2.對8對新鮮游離腓骨標(biāo)本相關(guān)數(shù)據(jù)進(jìn)行測量(男5對、女3對),應(yīng)用SPSS forWindows11.5統(tǒng)計軟件進(jìn)行處理,按得出的數(shù)據(jù)使用軟件進(jìn)行腓骨后方鋼板的建模。 結(jié)果: 1.自拆線后外固定應(yīng)用時間未見明顯差異;A組4例出現(xiàn)首次手術(shù)、1例出現(xiàn)再次手術(shù)取內(nèi)固定時腓骨外側(cè)切口破潰,經(jīng)換藥后重新縫合而愈合,B組未出現(xiàn)切口愈合問題,但4例發(fā)生腓骨肌腱炎;兩組AOFAS評分無明顯統(tǒng)計學(xué)差異,但B組AOFAS評分疼痛、功能、外觀和總分均優(yōu)于A組。 2.測量外踝與腓骨干的夾角、外踝長度及外踝尖至腓骨后面向內(nèi)旋轉(zhuǎn)臨界點的距離,結(jié)果提示前者以及后兩者之間的比值在不同的個體基本恒定。使用計算機(jī)圖形軟件按照測量結(jié)果對后側(cè)鋼板建模。 結(jié)論: 1.腓骨后側(cè)入路單切口治療累及腓骨下段和外踝的踝關(guān)節(jié)骨折,可以提高切口愈合率與骨折穩(wěn)定性,早期踝關(guān)節(jié)功能鍛煉,改善愈后。 2.腓骨后方內(nèi)固定物更符合腓骨的解剖結(jié)構(gòu),力學(xué)強(qiáng)度較腓骨外側(cè)鋼板牢靠。通過對腓骨遠(yuǎn)端的解剖結(jié)構(gòu)進(jìn)行測量,使用軟件模擬鋼板構(gòu)型,為設(shè)計更適合于腓骨后面的解剖鋼板提供了解剖和構(gòu)型依據(jù)。
