某醫(yī)科大學(xué)生自殺相關(guān)行為的心理、社會(huì)影響因素研究

【摘要】: [目的]了解大學(xué)生自殺相關(guān)行為的發(fā)生率,探討大學(xué)生自殺相關(guān)行為發(fā)生的心理、社會(huì)影響因素。 [方法]選擇某醫(yī)科大學(xué)能隨訪到的5 716名在校大學(xué)生作為研究對(duì)象。用自制的大學(xué)生心理健康狀況與相關(guān)生活經(jīng)歷調(diào)查表(包括自殺態(tài)度量表、自殺的可接受性量表、生活負(fù)性事件量表、家庭功能量表(APGAR)、生活質(zhì)量量表、Beck焦慮量表(BAI)、沖動(dòng)性量表(IMP)、攻擊性量表(AGG)、Beck抑郁量表(BDI)、社會(huì)支持量表(SSS)等)對(duì)目標(biāo)人群進(jìn)行調(diào)查。 [結(jié)果]大學(xué)生自殺意念率為13.9%,自殺未遂的發(fā)生率為1.3%。大學(xué)女生自殺意念的發(fā)生率為17.3%(464/2 679),男生的自殺意念率為10.8%(328/3 037),女生明顯高于男生,差異有顯著性。 大學(xué)生產(chǎn)生自殺意念的主要原因:工作學(xué)習(xí)問(wèn)題(38.0%)、家庭糾紛(19.6%)、情緒低落(12.4%)和戀愛(ài)問(wèn)題(10.1%);大學(xué)生自傷的主要方式為用工具(46.6%);自殺或自傷最主要的原因是:工作學(xué)習(xí)問(wèn)題(26.3%)、家庭糾紛(25.0%)、戀愛(ài)問(wèn)題(14.5%)、情緒低落(10.5%)及軀體疾病(10.5% );自殺未遂產(chǎn)生的前三位主要目的是:解脫痛苦(52.0%)、威脅他人(11.8% )、對(duì)現(xiàn)實(shí)的抗?fàn)?10.5% )。不同性別的大學(xué)生產(chǎn)生自殺意念的原因所占的比重不同,男生在戀愛(ài)問(wèn)題、經(jīng)濟(jì)困難、飲酒問(wèn)題上所占的比重高于女生;女生在家庭糾紛、與他人間的糾紛、工作和學(xué)習(xí)問(wèn)題上所占的比重則高于男生。 單因素分析結(jié)果表明,性別、專業(yè)、專業(yè)滿意度、年級(jí)、戀愛(ài)現(xiàn)狀、家庭經(jīng)濟(jì)狀況等6項(xiàng)因素與自殺意念的產(chǎn)生有關(guān)(P0.05);自殺態(tài)度、自殺的可接受性、生活負(fù)性事件、家庭功能、生命質(zhì)量、焦慮、抑郁等得分在有無(wú)自殺意念、自殺未遂的大學(xué)生中差異有顯著性(P0.05);不同性別的大學(xué)生在對(duì)自殺的可接受性、負(fù)性生活事件、家庭功能、生命質(zhì)量、焦慮、沖動(dòng)性和攻擊性、抑郁、社會(huì)支持各維度等得分在有無(wú)自殺意念的人中差異有顯著性(P0.05)。 多變量Logistic回歸分析結(jié)果表明,自殺意念與性別為男、負(fù)性生活事件、年齡、自殺未遂和自殺死亡的相似性、自殺的可預(yù)防性、對(duì)自殺的歧視、對(duì)自殺者的正性態(tài)度、家庭功能差、自殺的可接受性、焦慮、抑郁、社會(huì)支持程度高、攻擊性強(qiáng)等13項(xiàng)因素有關(guān)(P0.05);自殺未遂與專業(yè)滿意度差、高年級(jí)、性別為男、負(fù)性生活事件、對(duì)自殺的歧視、家庭功能差、自殺的可接受性、焦慮、攻擊性強(qiáng)等9項(xiàng)因素有關(guān)(P0.05)。不同年級(jí)、不同性別的大學(xué)生產(chǎn)生自殺意念的危險(xiǎn)因素間的差異很大。 [結(jié)論]大學(xué)生自殺相關(guān)行為主要影響因素包括性別、年齡、負(fù)性生活事件、對(duì)自殺的可接受性、家庭功能、生活質(zhì)量等。不同性別和年級(jí)的大學(xué)生自殺意念的影響因素差異較大,應(yīng)采取有針對(duì)性的預(yù)防大學(xué)生自殺的干預(yù)措施才能有效地降低大學(xué)生自殺相關(guān)行為的發(fā)生率。
[Abstract]:Objective: to investigate the incidence of suicide-related behaviors in college students and explore the psychological and social influencing factors of suicide-related behaviors. Methods: 5716 college students who were followed up in a medical university were selected as the subjects. The self-made questionnaire of college students' mental health status and related life experiences (including suicide attitude scale, suicide acceptability scale, life negative event scale) was used. Family function scale (APGAR), quality of Life scale (Bai), impulsive scale (IMP), aggression scale (AGG) Beck Depression scale (BDI), Social support scale (SSS) were investigated. [results] the rate of suicidal ideation and attempted suicide were 13.9% and 1.3% respectively. The incidence of suicidal ideation was 17.3% (464 / 2679) in college girls and 10.8% (328 / 3,037) in male students. The main causes of suicidal ideation in college students are: work-study problems (38.0%), family disputes (19.6%), depression (12.4%) and love problems (10.1%), the main ways of college students' self-injury are using tools (46.6%), and the main causes of suicide or self-injury are: work and study. Problems (26.3%), family disputes (25.0%), love problems (14.5%), depression (10.5%) and body diseases (10.5%), the first three main goals of attempted suicide were: relieving pain (52.0%), threatening others (11.8%), and fighting against reality (10.5%). College students of different genders account for different reasons for suicidal ideation. Male students account for a higher proportion in love problems, financial difficulties, drinking problems than girls, girls in family disputes, disputes with others, The proportion of work and study problems is higher than that of boys. Univariate analysis showed that six factors, such as gender, professional satisfaction, grade, current status of love, family economic status, were related to the emergence of suicidal ideation (P0.05), suicide attitude, acceptability of suicide, negative events of life, family function, etc. The scores of quality of life, anxiety and depression were significantly different among college students with or without suicidal ideation and attempted suicide (P0.05). The scores of impulsiveness and aggression, depression, social support were significantly different among people with or without suicidal ideation (P0.05). Multivariate logistic regression analysis showed that suicide ideation was similar to sex, negative life events, age, suicide attempt and suicide death, suicide was preventable, discrimination against suicide, positive attitude towards suicide. Family dysfunction, acceptability of suicide, anxiety, depression, high degree of social support and aggression were related to 13 factors (P0.05); attempted suicide was associated with poor professional satisfaction, senior grade, male sex, negative life events, and discrimination against suicide. There were 9 factors related to poor family function, acceptability of suicide, anxiety and aggression (P0.05). The risk factors of suicidal ideation in different grades and different genders are very different. [conclusion] the main influencing factors of suicide-related behaviors of college students include gender, age, negative life events, acceptability of suicide, family function, quality of life and so on. The influencing factors of suicidal ideation are different among different genders and grade students. The intervention measures should be taken in order to effectively reduce the incidence of suicide-related behaviors of college students.
