舌等離子射頻消融術(shù)與舌動脈及舌下神經(jīng)的解剖關系

【摘要】: 目的:通過對16側(cè)尸舌的局部解剖研究和測量舌動脈及舌下神經(jīng)在舌內(nèi)的走行位置觀察,采用比例的方法為舌等離子射頻消融術(shù)提供詳細解剖學方面的數(shù)據(jù)和理論依據(jù)。 材料及方法:1.標本來源:8具(16側(cè))福爾馬林固定、頜面部發(fā)育正常成人尸頭標本(不分性別、側(cè)別)。頸總動脈灌注紅色乳膠,鋸斷雙側(cè)下頜骨升支,開頜暴露舌表面及舌會厭谷,在正中聯(lián)合處將下頜骨鋸斷,自舌動脈于頸外動脈起點開始解剖舌動脈。 2試驗方法:對16側(cè)尸體舌的局部解剖測量,了解舌動脈及舌下神經(jīng)在舌內(nèi)的走行,并進行相對定位。 3.統(tǒng)計學分析:所測數(shù)據(jù)分別計算舌動脈及舌下神經(jīng)水平向距中線的距離與舌寬度及長度的比值,經(jīng)SPSS10.0軟件進行處理,行方差分析。 結(jié)果:舌動脈及舌下神經(jīng)的主干在舌盲孔周圍距舌表面的垂直距離不隨其走行而改變,約為20mm左右。舌動脈及舌下神經(jīng)水平位距中線距離與舌寬度的比值于舌盲孔后10 mm、舌盲孔及舌盲孔前10 mm分別是0.2692±0.0115、0.2623±0.0027、0.2334±0.0089、0.2619±0.0102、0.2021±0.0136、0.1925±0.0094。數(shù)據(jù)經(jīng)方差分析差異有統(tǒng)計學意義(P0.01)。 結(jié)論:舌根等離子射頻消融的相對安全范圍采用簡潔的比例算法更實用,即水平距離在舌中線1/3處,邊緣在外1/3,中間與外周1/3交點處最危險,表面下不應超過20mm。
[Abstract]:Objective: to study the local anatomy of 16 cadaveric tongue and observe the position of tongue artery and hypoglossal nerve in tongue, and to provide detailed anatomical data and theoretical basis for tongue plasma radiofrequency ablation by proportional method. Materials and methods: 1. The specimens were fixed with formalin (16 sides of 8 heads), and the heads of adult cadavers with normal maxillofacial development were fixed (regardless of sex and side). The common carotid artery was infused with red latex, and the ascending ramus of the bilateral mandible was sawed. The open jaw exposed the surface of the tongue and the valley of the epiglottis and sawed the mandible at the median junction and dissected the lingual artery from the origin of the external carotid artery. Methods: the local anatomic measurements of 16 cadaveric tongues were carried out to understand the movement of the lingual artery and hypoglossal nerve in the tongue and to locate them relatively. 3. Statistical analysis: the ratio of the distance between the lingual artery and the hypoglossal nerve to the median line and the width and length of the tongue were calculated. The data were processed by SPSS10.0 software and analyzed by ANOVA. Results: the vertical distance between the trunk of the lingual artery and the hypoglossal nerve from the lingual surface around the foramen caecum of the tongue did not change with the movement of the artery and the hypoglossal nerve. The ratio of the distance between the horizontal position of the lingual artery and the hypoglossal nerve to the median line and the width of the tongue was 10 mm behind the lingual caecal foramen, and 0.2692 鹵0.0115 ~ 0.2623 鹵0.0027 ~ 0.2334 鹵0.0089 ~ 0.2619 鹵0.01020.2021 鹵0.0136n 0.1925 鹵0.00944 for the tongue caecal foramen and 10 mm before the tongue caecal foramen, respectively. Conclusion: the relative safe range of radiofrequency ablation of tongue root is more practical, that is, the horizontal distance is 1/3 in the midline of tongue, the edge is 1 / 3 outside, and the intersection point between the middle and the peripheral 1/3 is the most dangerous, and the surface should not exceed 20mm.
