【摘要】: 目的 本研究旨在探討一種理想水壓的螺旋水刀在面神經(jīng)解剖術(shù)中的應(yīng)用。材料和方法 實驗動物為12只雜種狗,將其分為A、B兩組,每組6只。A組全部實驗狗均使用噴嘴直徑為120μm的螺旋水刀行保留面神經(jīng)的雙側(cè)腮腺全切術(shù),左側(cè)腮腺選用30-50 bar水壓,右側(cè)腮腺選用50-60 bar水壓。B組左側(cè)腮腺選用50-60bar水壓,噴嘴直徑為120μm的螺旋水刀施術(shù),右側(cè)腮腺選用常規(guī)手術(shù)器械施術(shù),雙側(cè)腮腺均行保留面神經(jīng)的雙側(cè)腮腺全切術(shù)。 A、B兩組觀察指標(biāo)相同。術(shù)前、術(shù)后觀察實驗狗的面神經(jīng)功能情況;術(shù)前、術(shù)后行面神經(jīng)電生理檢測;術(shù)后2周取面神經(jīng)標(biāo)本,行組織學(xué)觀察。 結(jié)果 實驗一(A組)結(jié)果:術(shù)前、術(shù)后所有實驗狗均未出現(xiàn)明顯面癱癥狀;行術(shù)后30-50 bar水壓組和50-60 bar水壓組的面神經(jīng)傳導(dǎo)速度比較,差別無統(tǒng)計學(xué)意義(P>0.05);行組織學(xué)觀察,兩組的神經(jīng)損傷均介于Ⅰ-Ⅱ級。 實驗二(B組)結(jié)果:術(shù)后常規(guī)手術(shù)器械組的部分實驗狗出現(xiàn)局部面癱癥狀;行術(shù)后50-60 bar水壓組和常規(guī)手術(shù)器械組的面神經(jīng)傳導(dǎo)速度比較,差別有統(tǒng)計學(xué)意義(P<0.05),前者速度大于后者;行組織學(xué)觀察,50-60 bar水壓組的神經(jīng)損傷介于Ⅰ-Ⅱ級,常規(guī)手術(shù)器械組的神經(jīng)損傷介于Ⅰ-Ⅳ級。 結(jié)論 本實驗證實:應(yīng)用螺旋水刀行面神經(jīng)解剖術(shù)可減小對面神經(jīng)的損害,較早恢復(fù)面神經(jīng)功能。
[Abstract]:Purpose. The purpose of this study was to investigate the application of an ideal water pressure spiral water knife in facial nerve dissection. Materials and methods. Twelve mongrel dogs were divided into two groups: group A (n = 6) and group A (n = 6). All dogs in group A were treated with a spiral water knife with a nozzle diameter of 120 渭 m. The left parotid gland was treated with 30 to 50 bar water pressure. In the right parotid gland, 50-60 bar water pressure was used in the left parotid gland, and the diameter of the nozzle was 120 渭 m in the left parotid gland. The right parotid gland was performed with conventional surgical instruments. The bilateral parotid gland was treated with total bilateral parotid gland with facial nerve preservation. The function of facial nerve in experimental dogs was observed before and after operation, the electrophysiological examination of facial nerve was performed before and after operation, and the specimens of facial nerve were taken 2 weeks after operation for histological observation. Results. Results: there were no obvious symptoms of facial paralysis in all dogs before and after operation, and there was no significant difference in the conduction velocity of facial nerve between the 30 to 50 bar water pressure group and 50-60 bar water pressure group (P > 0.05).
