脾動(dòng)脈解剖與其三維螺旋CT血管重建研究

【摘要】: 本研究通過脾動(dòng)脈CTA重建圖像,觀察研究了脾動(dòng)脈主干的走行及其與周圍組織關(guān)系。鑒于國內(nèi)尚缺乏脾動(dòng)脈三維重建的研究資料,對(duì)于脾動(dòng)脈與胰腺的解剖關(guān)系也無詳細(xì)研究,本研究通過影像學(xué)方法觀測(cè)脾動(dòng)脈的局部影像解剖資料以期為臨床胰腺外科手術(shù)、脾外科手術(shù)、腹腔鏡手術(shù)以及醫(yī)學(xué)影像學(xué)提供解剖學(xué)資料。 通過對(duì)脾動(dòng)脈主干的三維重建,清楚地顯示血管圖像以及與周圍組織的關(guān)系,研究中觀察到:脾動(dòng)脈主干及其分支走形較不規(guī)則,遠(yuǎn)端與胰尾、脾門關(guān)系密切且復(fù)雜。在腹部多種手術(shù)中需仔細(xì)處理脾動(dòng)脈的解剖關(guān)系,也是腹腔鏡手術(shù)中需要重點(diǎn)處理的內(nèi)容。但是,CTA對(duì)于研究脾葉動(dòng)脈及其分支尚不甚理想。VR能容易地重建脾動(dòng)脈主干及其較大的分支,但是由于需要人工設(shè)置CT值范圍,因此不能客觀地反映血管狹窄情況,臨床應(yīng)用受到限制。盡管如此,CTA能顯示脾動(dòng)脈及其周圍多個(gè)血管的關(guān)系、腹部內(nèi)臟器官的形態(tài)結(jié)構(gòu)、毗鄰關(guān)系,立體感強(qiáng)。隨著后處理軟件的發(fā)展,CTA將廣泛應(yīng)用于血管顯像及病變觀察,甚至有望替代DSA。
[Abstract]:In this study, CTA images of splenic artery were used to observe and study the course of splenic artery trunk and its relationship with surrounding tissues. In view of the lack of three-dimensional reconstruction of splenic artery in China, there was no detailed study on the anatomical relationship between splenic artery and pancreas. The purpose of this study was to provide anatomical data for clinical pancreatic surgery, splenic surgery, laparoscopic surgery and medical imaging. Through the three-dimensional reconstruction of the main trunk of the splenic artery, the images of the blood vessels and their relationship with the surrounding tissues were clearly displayed. In the study, it was observed that the main trunk of the splenic artery and its branches were irregular in shape, the distal part and the tail of the pancreas. The splenic portals are closely related and complex. The anatomical relationship of the splenic artery needs to be carefully handled in various abdominal operations. But CTA is not ideal for studying the splenic lobe artery and its branches. VR can easily reconstruct the splenic artery trunk and its larger branches, but because of the need to set the CT value range manually, Therefore, it can not objectively reflect the situation of vascular stenosis, and its clinical application is restricted. Nevertheless, CTA can show the relationship between splenic artery and its surrounding vessels, the shape and structure of abdominal visceral organs, and the adjacent relationship. With the development of post-processing software, CTA will be widely used in vascular imaging and pathological observation, and may even replace DSA.
