【摘要】： 本研究擬通過對10側(cè)新鮮成人下肢標(biāo)本的顯微解剖,對股深動(dòng)脈第一穿動(dòng)脈進(jìn)行系統(tǒng)觀測,以此支及伴行靜脈為血管蒂設(shè)計(jì)游離皮瓣,為四肢軟組織缺損的修復(fù)提供一種新的手術(shù)方法。 研究發(fā)現(xiàn),股深動(dòng)脈第一穿動(dòng)脈穿出大收肌后發(fā)出升支,第一穿動(dòng)脈主干向遠(yuǎn)端移行為降支。降支于距大轉(zhuǎn)子水平226.84mm±51.09mm處發(fā)出股后遠(yuǎn)端皮支,后者于距大轉(zhuǎn)子水平264.12mm±20.30mm處穿出深筋膜,穿出深筋膜時(shí)血管外徑為0.64mm±0.09mm。股后遠(yuǎn)端皮支自穿出深筋膜到分成上、下行支的長度為36.09mm±6.59mm。股深動(dòng)脈第一穿動(dòng)脈股后近端皮支的起源變異較多。 股深動(dòng)脈第一穿動(dòng)脈股后遠(yuǎn)端皮支有兩條伴行靜脈。二者匯入第一穿動(dòng)脈降支伴行靜脈,后者匯入第一穿動(dòng)脈伴行靜脈。 根據(jù)實(shí)驗(yàn)所得解剖數(shù)據(jù),設(shè)計(jì)以股深動(dòng)脈第一穿動(dòng)脈為血管蒂的游離皮瓣:軸心點(diǎn)位于股后中線距大轉(zhuǎn)子水平27cm及10cm處,軸心線為股后股骨髁間連線中點(diǎn)上方5cm向臀下垂直連線處,切取范圍為20cm×11cm。本課題設(shè)計(jì)的皮瓣為臨床修復(fù)軟組織缺損提供了一種新的手術(shù)方法,具有臨床應(yīng)用價(jià)值。
[Abstract]:In this study, the first perforating artery of deep femoral artery was systematically observed by microdissection of 10 fresh adult lower limb specimens, which was used to design free flap with venous pedicle. To provide a new surgical method for the repair of soft tissue defects in extremities. The study found that the first perforating artery of the deep femoral artery sent out the ascending branch after perforating the adductor Magnus. The first perforating artery moved to the distal end and the descending branch was located at 226.84 mm 鹵51.09 mm from the trochanter level. The latter perforated the deep fascia at a distance of 264.12mm 鹵20.30mm from the level of the greater trochanter. The external diameter of the vessel was 0.64 mm 鹵0.09 mm when the deep fascia was perforated. The cutaneous branch of the distal end of the femur perforated the deep fascia to the upper part of the fascia. The length of the descending branch was 36.09 mm 鹵6.59 mm. the origin of the proximal cutaneous branch of the deep femoral artery after the first perforating artery was more variable. There are two accompanying veins in the cutaneous branch of the posterior distal end of the deep femoral artery into the descending branch of the first perforating artery, and the latter into the first perforating artery with the vein. Based on the anatomical data obtained from the experiment, a free flap pedicled with the first perforating artery of deep femoral artery was designed. The axial point was located at 27cm and 10cm from the trochanter level. The axis line was 5 cm above the midpoint of the intercondylar line of the posterior femur and perpendicular to the bottom of the hip. The flap designed in this paper provides a new surgical method for clinical repair of soft tissue defects and has clinical application value.
