【摘要】: 目的:1.探討慢性應激與血壓、血糖和血脂的關系。2.研究慢性應激與血清皮質醇、腎上腺素和去甲腎上腺素的關系。3.評價慢性應激與5-HTTLPR基因多態(tài)性的關系。 方法:選擇駐守戈壁的某部軍人847人作為研究對象,使用特質應對方式問卷、領悟社會支持量表和90項癥狀清單評測其應激狀態(tài),測量收縮壓和舒張壓,測定空腹血糖、口服葡萄糖耐量實驗2h血糖和血清總膽固醇、甘油三酯、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇,檢測血清皮質醇、腎上腺素和去甲腎上腺素濃度,使用聚合酶鏈式反應技術和瓊脂糖凝膠電泳技術分析5-HTTLPR的基因多態(tài)性,將數(shù)據(jù)進行統(tǒng)計學處理。 結果:1.研究對象SCL-90高于SCL-90中國成人常模(P0.01);NC與FPG、OGTT2hPG、TC、1gTG和LDL-C存在線性關系(P0.05),PC與FPG存在線性關系(P0.05),SCL-90與SBP、FPG,OGTT2hPG、TC、lgTG和LDL-C存在線性關系(P0.05);NC是TC和LDL-C的影響因素(b=0.013,0.008),PC是FPG的影響因素(b=-0.006),SCL-90是SBP、OGTT2hPG、TC、1gTG和LDL-C的影響因素(b=0.014,0.002,0.002,0.001,0.001)。2.研究對象COR, E和NE與正常人測量值未見顯著差異(P0.05),SCL-90與COR、E和NE存在線性關系(P0.05),NC與COR和E存在線性關系(P0.05);NC和SCL-90是E的影響因素(b=0.385,0.054)。3.5-HTTLPR多態(tài)性的等位基因與研究對象的應激狀態(tài)相關,即不同等位基因的NC和SCL-90評分差異有統(tǒng)計學意義(χ2=21.232、30.359,P均小于0.05),攜帶S等位基因的個體應激水平較高;NC和SCL-90評分組Ⅰ高于組Ⅱ(P0.05),S/S基因型個體的應激水平高于S/L和L/L基因型個體。 結論:1.長期駐守戈壁的軍人應激水平高于正常人,慢性應激可能與高血壓、高血糖和高血脂的發(fā)病相關。2.慢性應激可能導致血清腎上腺素濃度升高。3.S等位基因相對L等位基因對應激易感,S/S基因型個體相對S/L和L/L基因型個體,可能產(chǎn)生較高應激水平。
[Abstract]:Objective 1. To investigate the relationship between chronic stress and blood pressure, blood glucose and blood lipid. 2. To study the relationship between chronic stress and serum cortisol. Relationship between epinephrine and norepinephrine. To evaluate the relationship between chronic stress and 5-HTTLPR gene polymorphism. Methods: a total of 847 soldiers stationed in Gobi were selected to evaluate their stress state with trait coping style questionnaire, perceived social support scale and 90 symptom lists. Systolic blood pressure (SBP) and diastolic blood pressure (DBP), fasting blood glucose (FBG), 2 h glucose level in oral glucose tolerance test and serum total cholesterol, triglyceride, low density lipoprotein cholesterol and high density lipoprotein cholesterol were measured. Serum cortisol, epinephrine and norepinephrine concentrations were detected. The gene polymorphisms of 5-HTTLPR were analyzed by polymerase chain reaction and agarose gel electrophoresis. Statistic the data. Results 1. The SCL-90 of the subjects was higher than that of SCL-90 Chinese adult norm (P0.01A). There was a linear relationship between NC and OGTT _ 2hPGT _ 2g TG and LDL-C. There was a linear relationship between PC and FPG (P _ (0.05)). There was a linear relationship between SCL-90 and OGTT2hPGG TG and LDL-C. NC is the influencing factor of TC and LDL-C. PC is the influence factor of FPG. SCL-90 is SBP. The influencing factors of 1 g TG and LDL-C of OGTT2hPGN were 0.014 ~ 0.002 ~ 0.002 ~ 0. 001 ~ 0. 001. There was no significant difference between the measured values of Corr, E and NE and that of normal subjects (P 0.05), SCL-90 and COR. There is a linear relationship between E and NE. There is a linear relationship between NC and COR and E. The alleles of NC and SCL-90 were the influencing factors of E, the alleles of 0.385 and 0.054. 3.5-HTTLPR polymorphism were associated with the stress state of the subjects. The scores of NC and SCL-90 of different alleles were significantly different (蠂 2, 21.232, 30.359, P < 0.05).
