【摘要】：目的:本研究旨在了解2015年度遼寧省精神衛(wèi)生資源狀況及分布特點(diǎn),發(fā)現(xiàn)遼寧省精神衛(wèi)生資源的優(yōu)點(diǎn)與不足,分析存在問題及原因,積極采取應(yīng)對(duì)措施,為優(yōu)化遼寧省精神衛(wèi)生資源配置,制定精神衛(wèi)生政策提供科學(xué)依據(jù),為促進(jìn)精神衛(wèi)生事業(yè)的發(fā)展貢獻(xiàn)力量。方法:采用問卷調(diào)查對(duì)遼寧省2015年精神衛(wèi)生�？茩C(jī)構(gòu)和精神衛(wèi)生防治機(jī)構(gòu)狀況進(jìn)行調(diào)查,主要收集其基礎(chǔ)信息、床位資源和人力資源狀況,利用EXCEL軟件進(jìn)行描述性分析,采用繪制洛倫茲曲線和計(jì)算Gini系數(shù)來(lái)分析各市相關(guān)資源分配的公平性。結(jié)果:1.遼寧省2015年擁有精神衛(wèi)生�？茩C(jī)構(gòu)114所,其中公立機(jī)構(gòu)83(72.81%)所,民營(yíng)機(jī)構(gòu)31(27.19%)所,精神衛(wèi)生專科機(jī)構(gòu)覆蓋率69.00%;精神衛(wèi)生機(jī)構(gòu)最多的地級(jí)市為沈陽(yáng)市、鐵嶺市、大連市。2.精神衛(wèi)生�？茩C(jī)構(gòu)共有衛(wèi)生人員14899人,其中精神衛(wèi)生專業(yè)技術(shù)人員7177人,占48.17%;精神科執(zhí)業(yè)醫(yī)師1575人,人口密度為3.71人/10萬(wàn)人口;精神衛(wèi)生專科機(jī)構(gòu)中共有醫(yī)生數(shù)2116人,按人口分布和地理面積分布的Gini系數(shù)分別為0.123981和0.247029;精神科護(hù)士4425人,護(hù)士人口密度為10.43人/10萬(wàn)人口,按人口分布和地理面積分布的Gini系數(shù)分別為0.122976和0.274814;其他人員包括醫(yī)技344人,心理治療師164人,心理咨詢師530人,社會(huì)工作者55人。3.全部精神衛(wèi)生�？茩C(jī)構(gòu)中開設(shè)病房的由90家,編制床位數(shù)18022張,實(shí)際開放床位數(shù)24901張,人口密度為5.867張/萬(wàn)人,按人口分布和地理面積分布的Gini系數(shù)分別為0.166938和0.285294;我省精神科平均住院日數(shù)103.17天/人,平均月住院費(fèi)用3387.69元/人;我省精神�？茩C(jī)構(gòu)收治入院患者主要以精神分裂癥為主,占總住院人數(shù)的72.73%;2015年遼寧省精神衛(wèi)生�？茩C(jī)構(gòu)總計(jì)撥款18686.27萬(wàn)元,總收入131972.21萬(wàn)元。4.精神衛(wèi)生防治機(jī)構(gòu)(精神衛(wèi)生防治辦公室)115家,獨(dú)立編制2家,其余均為掛靠機(jī)構(gòu),掛靠�？漆t(yī)院28家,疾病預(yù)防控制中心65家,其他機(jī)構(gòu)20家。結(jié)論:遼寧省精神衛(wèi)生資源處于全國(guó)前列,在數(shù)量上達(dá)到中等資源配置,但機(jī)構(gòu)分布不均,人力資源相對(duì)缺乏,結(jié)構(gòu)不合理,特別是精神衛(wèi)生防治人員多為兼職,精神衛(wèi)生知識(shí)缺乏,防治技術(shù)不足;在未來(lái)遼寧省應(yīng)注重資源的合理布局,增加精神衛(wèi)生投入,規(guī)范精神衛(wèi)生人員培養(yǎng)與配置。
[Abstract]:Objective: the purpose of this study was to understand the status and distribution of mental health resources in Liaoning Province in 2015, to find out the advantages and disadvantages of mental health resources in Liaoning Province, to analyze the existing problems and reasons, and to take active measures to deal with them. In order to optimize the allocation of mental health resources and formulate mental health policies in Liaoning Province, it can provide scientific basis and contribute to the development of mental health. Methods: a questionnaire survey was conducted on mental health institutions and mental health prevention institutions in Liaoning province in 2015. The basic information, bed resources and human resources were collected and analyzed by EXCEL software. Lorentz curve and Gini coefficient are used to analyze the fairness of resource allocation. The result is 1: 1. There are 114 mental health institutions in Liaoning province in 2015, of which 83 (72.81%) are public institutions, 31 (27.19%) are private institutions, and 69.00 are mental health institutions. The most prefecture-level cities with mental health institutions are Shenyang, Tieling and Dalian. The total number of health personnel in mental health specialized institutions is 14899, including 7177 mental health professionals and technicians, 1575 psychiatrists with a population density of 3.71 people / 100000 population, and 2116 doctors in mental health specialized institutions. The Gini coefficients of population distribution and geographical area distribution were 0.123981 and 0.247029 respectively, 4425 psychiatric nurses, 10.43 nurses / 100000 population, 0.122976 and 0.274814 Gini coefficients according to population distribution and geographical area distribution, respectively. 164 psychotherapists, 530 counsellors and 55 social workers. Of the total mental health institutions, 90 had wards, 18022 beds were compiled, 24901 beds were actually opened, and the population density was 5.867 beds per 10, 000 people. The Gini coefficients according to population distribution and geographical area distribution were 0.166938 and 0.285294.The average number of hospitalization days per person was 103.17 days per person, and the average monthly hospitalization cost was 3387.69 yuan per person. The total allocation of mental health institutions in Liaoning Province in 2015 was 186.8627 million yuan, with a total income of 1.3197221 billion yuan. There were 115 mental health prevention and control institutions (mental health prevention and control office), 2 were independently compiled, the rest were affiliated institutions, 28 affiliated specialized hospitals, 65 centers for disease prevention and control, and 20 other institutions. Conclusion: the mental health resources in Liaoning Province are in the forefront of the whole country and reach the middle resource allocation in quantity, but the organization distribution is uneven, the human resource is relatively lacking, the structure is unreasonable, especially the mental health prevention and treatment personnel are part-time. In the future Liaoning Province should pay attention to the rational distribution of resources increase the input of mental health and standardize the cultivation and allocation of mental health personnel.
