【摘要】：為構(gòu)建適合我國國情的藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)工具,深入了解實(shí)驗(yàn)室的能力現(xiàn)狀及存在的問題,并為加強(qiáng)能力建設(shè)從而提出有針對(duì)性的政策建議提供數(shù)據(jù)支撐,有必要將藥品檢驗(yàn)實(shí)驗(yàn)室的能力進(jìn)行量性研究,構(gòu)建一個(gè)科學(xué)合理的"藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系",建立相應(yīng)的評(píng)價(jià)標(biāo)準(zhǔn)及評(píng)價(jià)方式,從而科學(xué)、有效、充分地評(píng)價(jià)藥品檢驗(yàn)實(shí)驗(yàn)室的綜合能力。本研究通過文獻(xiàn)綜述和專家訪談法,總結(jié)歸納藥品檢驗(yàn)實(shí)驗(yàn)室能力的核心構(gòu)成要素,初步構(gòu)建實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系的基本框架;采取兩輪德爾菲專家咨詢對(duì)評(píng)價(jià)指標(biāo)體系進(jìn)行修改和完善;運(yùn)用層次分析法確定各級(jí)指標(biāo)權(quán)重,構(gòu)建藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)綜合模型,并對(duì)該模型進(jìn)行信度和效度檢驗(yàn);利用建立的藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系和評(píng)價(jià)模型對(duì)青海省7個(gè)實(shí)驗(yàn)室進(jìn)行實(shí)證性研究,并對(duì)調(diào)查及評(píng)價(jià)結(jié)果進(jìn)行匯總分析。本研究構(gòu)建的藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系,包括資源配置和技術(shù)能力2個(gè)一級(jí)指標(biāo),人力資源、儀器設(shè)備、基礎(chǔ)設(shè)施、信息化建設(shè)、檢驗(yàn)?zāi)芰�、檢驗(yàn)工作量、能力驗(yàn)證和科研能力8個(gè)二級(jí)指標(biāo)以及23個(gè)三級(jí)指標(biāo);建立的綜合評(píng)價(jià)模型為Y=ΣW·Yi=(0.0800Y1+0.0441Y2+0.0243Y3+0.0655Y4+…+0.0319Y23)*10;選取青海省兩級(jí)共 7個(gè)藥品檢驗(yàn)機(jī)構(gòu)作為指標(biāo)體系的實(shí)證研究對(duì)象,研究結(jié)果為:青海省各藥品檢驗(yàn)機(jī)構(gòu)能力綜合評(píng)分介于28.13-66.11分之間,省院得分明顯高于州(市)級(jí)藥品檢驗(yàn)機(jī)構(gòu),州(市)級(jí)藥品檢驗(yàn)機(jī)構(gòu)中,實(shí)驗(yàn)室B得分最高為39.38分,實(shí)驗(yàn)室D得分最低為28.13分;從8個(gè)二級(jí)指標(biāo)的原始得分來看,能力驗(yàn)證和檢驗(yàn)?zāi)芰Φ梅州^高,信息化建設(shè)和檢驗(yàn)工作量得分較低,成為導(dǎo)致綜合能力較低的短板。本研究首次站在現(xiàn)代管理體系的平臺(tái)上,構(gòu)建了具有良好信、效度的藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系,評(píng)價(jià)結(jié)果可為實(shí)驗(yàn)室自身和監(jiān)管者所用,為公正、客觀、全面地評(píng)價(jià)藥品檢驗(yàn)實(shí)驗(yàn)室的能力提供了工具;通過應(yīng)用構(gòu)建的藥品檢驗(yàn)實(shí)驗(yàn)室能力評(píng)價(jià)指標(biāo)體系,可以深入分析當(dāng)前藥品檢驗(yàn)實(shí)驗(yàn)室能力的現(xiàn)狀,并可依據(jù)評(píng)價(jià)的結(jié)果,調(diào)整資源配置方向,加強(qiáng)能力建設(shè),全面提升綜合能力。
[Abstract]:In order to build the ability evaluation tool suitable for the national conditions of our country, to understand the present situation and existing problems of the laboratory, and to provide the data support for the strengthening of the capacity building so as to put forward pertinent policy suggestions, it is necessary to carry out a quantitative study on the ability of the drug inspection laboratory and build a scientific and reasonable "medicine". It is scientific, effective, and fully appraising the comprehensive ability of the drug inspection laboratory. This study summarizes the core components of the power of the laboratory laboratory through literature review and expert interview. The basic framework of the index system; adopting two rounds of Delphy expert consultation to modify and perfect the evaluation index system; use the analytic hierarchy process to determine the weight of the indicators at all levels, construct the comprehensive model of the ability evaluation of the laboratory for drug inspection, and test the reliability and validity of the model, and use the established evaluation index of the ability of the drug inspection laboratory. The system and evaluation model have carried out an empirical study on 7 laboratories in Qinghai Province, and the results of the investigation and evaluation are summarized and analyzed. The evaluation index system of the capacity of the drug inspection laboratory is constructed, including the 2 first level indexes of the resource allocation and technical ability, the human resources, the equipment, the infrastructure, the information construction, the inspection ability and the inspection. The test workload, capacity verification and scientific research capacity 8 two level indicators and 23 three level indicators; the established comprehensive evaluation model is Y= Sigma W. Yi= (0.0800Y1+0.0441Y2+0.0243Y3+0.0655Y4+... +0.0319Y23) *10; selected 7 drug inspection institutions in Qinghai province two as the target system of the empirical research. The results are as follows: the comprehensive score of the drug inspection institutions in Qinghai is between the 28.13-66.11 points, the provincial hospital scores are obviously higher than the state (city) drug inspection institutions, and the state (city) level drug inspection institutions are the most successful in the laboratory. At 39.38 points, the lowest score of D in the laboratory is 28.13. From the original score of the 8 two level indicators, the ability to verify and test the ability is higher, the information construction and the test work score is low, which has become a short board which leads to the low comprehensive ability. This research has built a good letter and validity on the platform of modern management system for the first time. The evaluation index system of drug test laboratory capacity can be used by the laboratory and the supervisor. It provides a tool for the impartiality, objective and comprehensive evaluation of the ability of the drug inspection laboratory. Through the application and construction of the capability evaluation index system of the drug inspection laboratory, the ability of the current drug inspection laboratory can be deeply analyzed. Based on the evaluation results, we should adjust the direction of resource allocation, strengthen capacity building, and comprehensively enhance the comprehensive ability.
