橋小腦角區(qū)血管神經(jīng)復(fù)合體的解剖學(xué)研究

【摘要】:目的研究橋小腦角區(qū)血管神經(jīng)間的解剖關(guān)系,為臨床微血管減壓術(shù)提供解剖學(xué)資料。方法20例10%甲醛固定并灌有乳膠的成人頭顱濕標(biāo)本,去除大腦半球、小腦幕和模擬臨床枕下乙狀竇后手術(shù)入路,逐層解剖、觀察、照相。方法橋小腦角區(qū)上血管神經(jīng)復(fù)合體主要包括滑車神經(jīng)、三叉神經(jīng)和小腦上動(dòng)脈、小腦下前動(dòng)脈、巖上靜脈、巖上竇等。小腦上動(dòng)脈主干尾袢及其分叉部附近常壓迫三叉神經(jīng)出入腦干區(qū)的上方。中血管神經(jīng)復(fù)合體主要包括展神經(jīng)、面聽神經(jīng)和小腦下前動(dòng)脈、小腦下后動(dòng)脈等。小腦下前動(dòng)脈常呈折疊狀凸入內(nèi)耳,在折疊端及其附近常發(fā)出內(nèi)聽動(dòng)脈入內(nèi)耳。下血管神經(jīng)復(fù)合體主要包括延髓、后組顱神經(jīng)、舌下神經(jīng)、小腦下后動(dòng)脈、椎動(dòng)脈等。結(jié)論責(zé)任血管可能不止一根,壓迫點(diǎn)可能不止一處,詳盡的解剖研究可提高顯微血管減壓手術(shù)的成功率。
[Abstract]:Objective to study the anatomical relationship between blood vessels and nerves in cerebellopontine angle region and to provide anatomical data for clinical microvascular decompression. Methods 20 cases of adult head wet specimens fixed with 10% formaldehyde and latex were removed from cerebral hemisphere, cerebellar tentorium and simulated posterior occipital sigmoid sinus approach, dissected layer by layer, observed and photographed. Methods the superior vascular nerve complex in cerebellopontine angle mainly included trocar nerve, trigeminal nerve and superior cerebellar artery, anterior inferior cerebellar artery, superior petrosal vein, superior petrosal sinus and so on. The trigeminal nerve is often pressed into and out of the upper part of the trunk and trunk of the superior cerebellar artery near the tail loop and its bifurcation. The middle vascular nerve complex mainly includes abductor nerve, facial auditory nerve and anterior inferior cerebellar artery, posterior inferior cerebellar artery and so on. The anterior inferior cerebellar artery is often folded into the inner ear, and the inner auditory artery is often sent into the inner ear at and near the folded end. The inferior vascular nerve complex mainly includes medulla oblongata, posterior group skull nerve, hypoglossal nerve, inferior cerebellar posterior artery, vertebral artery and so on. Conclusion there may be more than one responsible vessel and more than one compression point. Detailed anatomical study can improve the success rate of microvascular decompression.
