中國人群主動脈根部結(jié)構(gòu)及生理影響因素研究

【摘要】: 研究目的和意義:探討性別、年齡、身高、體重、體表指數(shù)BMI、體表面積BSA、收縮壓、舒張壓及脈壓等生理因素對中國人群主動脈竇部、環(huán)部及弓部結(jié)構(gòu)的影響。 研究內(nèi)容和方法: 1.重復性研究 入選2007年9月至12月鹽城市第一人民醫(yī)院住院病人共11例,排除標準:擴張性心肌病,肥厚性心肌病,心肌炎,先天性心臟病,主動脈瓣膜病變,主動脈竇瘤,主動脈縮窄,主動脈弓部畸形及主動脈瓣置換術(shù)后病人均不在入選范圍之內(nèi)。通過PHILIP - 7500型彩色多普勒顯像儀二維超聲測量主動脈瓣環(huán)部、竇部及弓部的內(nèi)徑。同一名檢測醫(yī)生在不同時間段先后三次,對11名受試者進行測量。三名檢測醫(yī)生在不同時間段,各自分別對11名受試者進行測量。 由SPSS13.0軟件包進行重復試驗的信度和內(nèi)部相關(guān)一致性進行檢驗。 2.相關(guān)性研究 入選2007年9月至12月鹽城市第一人民醫(yī)院住院病人共748例,排除標準:擴張性心肌病,肥厚性心肌病,心肌炎,先天性心臟病,主動脈瓣膜病變,主動脈竇瘤,主動脈縮窄,主動脈弓部畸形及主動脈瓣置換術(shù)后病人均不在入選范圍之內(nèi)。有效數(shù)據(jù)715例,有效比例95.6%。 通過PHILIP - 7500型彩色多普勒顯像儀對受試者進行二維超聲測量主動脈瓣環(huán)部、竇部及弓部的內(nèi)徑,并且記錄受試者性別、身高、體重、體表指數(shù)BMI、體表面積BSA、收縮壓、舒張壓及脈壓。每個對象測量3次,每次測量都在ECG同一位置進行超聲圖象存檔,相關(guān)軟件進行測量分析記錄。 由SPSS13.0軟件包采用多元逐步回歸法分析得出相關(guān)因素及其回歸方程,并作出散點圖。 研究結(jié)果: 1.與主動脈竇部直徑關(guān)系最重要的因素是年齡,其次是體表面積BSA、性別、和舒張壓,其中與性別負相關(guān);貧w方程為竇=17.148+4.553XBSA+0.056X年齡-1.276X性別+0.027X舒張壓 2.與主動脈環(huán)部直徑關(guān)系最重要的因素是體表面積BSA,其次是性別、年齡,其中與性別負相關(guān);貧w方程為環(huán)=12.565+4.690XBSA-1.053X性別+0.022X年齡 3.與主動脈弓部直徑關(guān)系的因素是年齡和體表面積BSA;貧w方程為弓=13.645+4.694XBSA+0.051X年齡 結(jié)論: 1.主動脈竇部,瓣環(huán)及弓部直徑有顯著的性別差異,在竇部及環(huán)部可以用性別引起的體表面積差異解釋,但弓部尚不能用性別引起的體表面積差異解釋。 2.體表面積是主動脈竇部,瓣環(huán)部及弓部結(jié)構(gòu)重要影響因素。隨著體表面積增加而升高。體表面積是影響主動脈環(huán)部直徑大小最重要的影響因素。 3.年齡是主動脈竇部,瓣環(huán)部及弓部重要影響因素。隨著年齡增長而擴大。年齡是影響主動脈竇部及弓部直徑大小最重要的影響因素 4.主動脈竇部直徑大小隨著舒張壓升高而升高。舒張壓是一個相對次要的影響因素。
[Abstract]:Objective and significance: to investigate the effects of physiological factors such as sex, age, height, weight, body surface index BMI, body surface area BSA, systolic blood pressure, diastolic blood pressure and pulse pressure on aortic sinus, ring and arch structure in Chinese population. Research contents and methods: 1. From September to December 2007, 11 inpatients in Yancheng first people's Hospital were enrolled in the repetitive study. The exclusion criteria were dilated cardiomyopathy, hypertrophic cardiomyopathy, myocarditis, congenital heart disease and aortic valvular disease. Aortic sinus aneurysm, aortic coarctation, aortic arch malformation and aortic valve replacement were not included. The inner diameters of aortic valve ring, sinus and arch were measured by PHILIP-7500 color Doppler imaging. The same examiner measured 11 subjects three times in different periods. The three doctors measured 11 subjects at different time periods. The reliability and internal consistency of repeated tests were tested by SPSS13.0 software package. 2. A total of 748 inpatients in Yancheng first people's Hospital from September to December 2007 were enrolled in the study. The exclusion criteria were dilated cardiomyopathy, hypertrophic cardiomyopathy, myocarditis, congenital heart disease and aortic valvular disease. Aortic sinus aneurysm, aortic coarctation, aortic arch malformation and aortic valve replacement were not included. The effective data were 715 cases, the effective rate was 95.6%. The inner diameters of aortic annulus, sinus and arch were measured by two-dimensional ultrasound with PHILIP-7500 color Doppler imager. the gender, height, weight and body surface index BMI, surface area BSA, systolic blood pressure were recorded. Diastolic pressure and pulse pressure. Each object was measured three times, and the ultrasonic image was archived in the same location of ECG, and the related software was used for measurement and analysis. By using SPSS13.0 software package, the related factors and their regression equations are analyzed by multivariate stepwise regression method, and the scattered point diagram is made. Results: 1. The most important factor related to aortic sinus diameter was age, followed by body surface area BSA, sex and diastolic blood pressure, which was negatively correlated with sex. The regression equation was sinus = 17.148 4.553XBSA 0.056X age-1.276X sex 0.027X diastolic blood pressure. The most important factor related to the diameter of aortic ring was that body surface area BSA, was followed by sex and age, which was negatively correlated with sex. The regression equation is ring = 12.565 4.690XBSA-1.053X sex 0.022X age. The factors related to the diameter of aortic arch are age and body surface area BSA.. The regression equation is arch = 13.645 4.694XBSA 0.051X age conclusion: 1. There was significant gender difference in the diameter of aortic sinus, valvular ring and arch. The difference of body surface area caused by sex could be explained in sinus and ring, but the difference of body surface area caused by sex could not be explained in arch. 2. Body surface area is an important factor affecting the structure of aortic sinus, valve ring and arch. It increases with the increase of body surface area. Body surface area is the most important factor affecting the diameter of aortic ring. 3. Age is an important influencing factor in aortic sinus, valvular ring and arch. It expands with age. Age is the most important factor affecting the diameter of aortic sinus and arch. The diameter of aortic sinus increased with the increase of diastolic pressure. Diastolic blood pressure is a relatively secondary factor.
