【摘要】： 目前全球糖尿病患者已達(dá)1.8億左右,以北京為例,北京市的糖尿病發(fā)病率為6%,糖尿病人超過80萬人。糖尿病的患病率、致殘率和病死率以及對健康的總體危害程度,已居北京市慢性非傳染性疾病的第3位。糖尿病作為一種身心疾病,是現(xiàn)代醫(yī)學(xué)無法治愈的難題。長期服用降糖藥、注射胰島素的治療方法,已經(jīng)實(shí)現(xiàn)了降低血糖、控制數(shù)值的目的。但治療不等于治愈,治療僅僅等于降低血糖值。隨著近年來“生物-心理-社會(huì)醫(yī)學(xué)模式”的提出,糖尿病的心理干預(yù)給糖尿病的研究帶來了一種新的思路。 本研究欲采用國內(nèi)一項(xiàng)專門的心理咨詢技術(shù)——意象對話心理咨詢與治療技術(shù)(Imagery Communication Psychotherapy),對二型糖尿病患者進(jìn)行心理干預(yù)與治療。研究欲通過意象對話心理咨詢,直觀的呈現(xiàn)患者心理狀態(tài)中與糖尿病有關(guān)部分;再通過治療提高這部分的心理健康水平,并觀察意象對話心理干預(yù)對血糖的影響。研究通過記錄患者治療前后空腹血糖、餐后兩小時(shí)血糖、糖化血紅蛋白三個(gè)指標(biāo)的變化,對患者進(jìn)行前、后測效果檢驗(yàn)。結(jié)果發(fā)現(xiàn),同接受意象對話治療之前相比,這三個(gè)血液指標(biāo)都有所下降,但只有空腹血糖降低達(dá)到顯著水平(P〈0.05)。而患者心理呈現(xiàn)出過分好強(qiáng)和缺乏愛與被愛能力這兩個(gè)特點(diǎn)。
[Abstract]:At present, the number of diabetes patients in the world has reached about 180 million. Take Beijing as an example, the incidence of diabetes in Beijing is 6%, and the number of diabetic patients exceeds eight hundred thousand. The prevalence, disability, mortality and overall health hazards of diabetes are the third most common chronic non-communicable diseases in Beijing. Diabetes, as a physical and mental disease, is a difficult problem that cannot be cured in modern medicine. Long-term use of hypoglycemic drugs, insulin injection, has achieved the goal of reducing blood sugar and controlling the value. But treatment is not a cure, treatment is merely a reduction in blood sugar. With the development of "bio-psycho-social medical model" in recent years, the psychological intervention of diabetes brings a new thought to the study of diabetes mellitus. In this study, a special psychological counseling technique, image dialogue counseling and treatment technique (Imagery Communication Psychotherapy), was used for psychological intervention and treatment of type 2 diabetes mellitus (DM) patients. The purpose of this study is to visualize the mental state of patients with diabetes mellitus through image dialogue psychological consultation, and then to improve the mental health level of this part through treatment, and to observe the effect of mental intervention on blood sugar by image dialogue psychological intervention. The changes of fasting blood glucose, postprandial blood glucose and glycosylated hemoglobin (HbA1c) were recorded before and after treatment, and the results were tested before and after treatment. The results showed that compared with those before the treatment of image dialogue, all the three blood indexes decreased, but only fasting blood glucose decreased significantly (P < 0.05). The patients' psychology shows two characteristics: excessive strength and lack of love and being loved ability.
