PBC患者肝內(nèi)和外周血淋巴細(xì)胞CCR5、CXCR3、CXCR6和CXCR7的表達(dá)及意義

【摘要】: 目的比較原發(fā)性膽汁性肝硬化(primary biliary cirrhosis,PBC)患者肝內(nèi)和外周血淋巴細(xì)胞上趨化因子受體CCR5、CXCR3、CXCR6和CXCR7的表達(dá)水平,及其與肝臟組織學(xué)炎癥反應(yīng)的關(guān)系,在趨化因子及其受體領(lǐng)域上初步探討原發(fā)性膽汁性肝硬化的發(fā)病機(jī)制。 方法本研究同時(shí)收集來自昆明醫(yī)學(xué)院第二附屬醫(yī)院肝膽胰內(nèi)科2008年2月~2008年12月住院并確診的11例PBC患者肝組織活檢標(biāo)本和靜脈血樣本。應(yīng)用Beckman Coulter Epics XL流式細(xì)胞儀檢測肝組織和外周血淋巴細(xì)胞CCR5、CXCR3、CXCR6和CXCR7的表達(dá)情況,并進(jìn)行對比分析。 結(jié)果1、一般資料原發(fā)性膽汁性肝硬化患者11例,平均年齡43.3±14.6歲,女7例,男4例。其中原發(fā)性膽汁性肝硬化達(dá)Ⅲ-Ⅳ期和Ⅳ期共7例,Ⅱ期及以下為4例。 2、肝組織浸潤淋巴細(xì)胞以CD8+、CD4+T淋巴細(xì)胞為主;外周血中CD4+T淋巴細(xì)胞明顯占優(yōu)勢。肝臟組織中CD8+T淋巴細(xì)胞高于外周血,而肝組織中B淋巴細(xì)胞與外周血比較無統(tǒng)計(jì)學(xué)差異(P>0.05)。外周血中CD4+T淋巴細(xì)胞高于肝組織(P<0.05)。而不同肝組織學(xué)炎癥程度組之間上述淋巴細(xì)胞亞群浸潤比較均無統(tǒng)計(jì)學(xué)差異(P>0.05)。 3、肝組織內(nèi)表達(dá)CCR5、CXCR3、CXCR6明顯高于外周血(P<0.001),CXCR7在肝組織內(nèi)表達(dá)水平要明顯低于CCR5、CXCR3和CXCR6,但其在肝組織內(nèi)表達(dá)仍高于外周血(P<0.05)。 4、肝臟病變程度嚴(yán)重組的肝組織內(nèi)CXCR6表達(dá)水平要高于病變輕微組(P<0.05),CCR5、CXCR3和CXCR7的表達(dá)水平無統(tǒng)計(jì)學(xué)差異(P>0.05)。 5、肝組織和外周血表達(dá)細(xì)胞因子IL-2、IL-4、IL-5、IL-10、IFN-γ和TNF-α水平均明顯增高,但后者在兩組之間比較均無統(tǒng)計(jì)學(xué)差異(P>0.05)。 結(jié)論1、PBC與機(jī)體免疫功能異常密切相關(guān),并且CD8+T細(xì)胞、CD4+T細(xì)胞介導(dǎo)的免疫病理反應(yīng)起比較重要的作用,可能是自身免疫反應(yīng)中靶器官——膽管上皮細(xì)胞損傷的重要機(jī)制。 2、趨化因子受體CCR5、CXCR3、CXCR6可能介導(dǎo)T淋巴細(xì)胞向肝內(nèi)定植,并參與PBC肝臟炎癥的病理免疫學(xué)反應(yīng)過程。 3、趨化因子受體CXCR6在介導(dǎo)T淋巴細(xì)胞向肝內(nèi)定植,參與肝臟炎癥的病理免疫學(xué)反應(yīng)過程中可能發(fā)揮更為重要的作用。 4、CXCR7的介導(dǎo)趨化作用輕微,可能不是調(diào)節(jié)T淋巴細(xì)胞向肝內(nèi)定植的主要相關(guān)趨化因子受體。 5、細(xì)胞因子特別是IL-2、IFN-γ可能參與PBC病理形成過程。
