慢性HBV感染者外周血調(diào)節(jié)性T細(xì)胞水平及Foxp3與CD127的表達(dá)關(guān)系

【摘要】: 目的: 探討慢性HBV感染者外周血中CD4+CD25+Foxp3+調(diào)節(jié)性T細(xì)胞(Regulatory T,Treg)的水平,并結(jié)合感染者的臨床資料,分析Treg在HBV感染慢性化機(jī)制中的作用和與疾病進(jìn)展的相關(guān)性,并初步研究其兩種細(xì)胞標(biāo)志—CD127分子與Foxp3轉(zhuǎn)錄因子表達(dá)之間的關(guān)系。 方法: 采集34例免疫耐受期、26例免疫清除期和31例非活動(dòng)或低(非)復(fù)制期慢性HBV感染者的外周全血,采用流式細(xì)胞染色技術(shù),通過(guò)對(duì)CD4、CD25、Foxp3和CD127等細(xì)胞表面和內(nèi)部標(biāo)志進(jìn)行熒光染色,用流式細(xì)胞分析儀設(shè)門(mén)圈定細(xì)胞群,檢測(cè)CD4+CD25+Foxp3+ T細(xì)胞和CD4+CD25+CD127low T細(xì)胞的頻率。 結(jié)果: 1.在CD4+CD25+Foxp3+ T細(xì)胞占CD4+ T細(xì)胞的百分比中,免疫耐受組的比例(4.17±0.75)%高于非活動(dòng)或低(非)復(fù)制組(3.66±0.85)%,差異具有統(tǒng)計(jì)學(xué)意義(Z=-2.693,P=0.007);且病毒陽(yáng)性組(免疫耐受組+免疫清除組,HBeAg陽(yáng)性,60例,(4.09±0.86)%)的比例高于病毒陰性組(非活動(dòng)或低(非)復(fù)制組,HBeAg陰性,31例,(3.66±0.85)%)(t=2.266,P=0.026),差異具有統(tǒng)計(jì)學(xué)意義。將ALT異常組(免疫清除組)與ALT正常組(免疫耐受組+非活動(dòng)或低(非)復(fù)制組,65例)的比例進(jìn)行比較,結(jié)果顯示兩組的差異(3.99±1.00)%與(3.93±0.83)%之間無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);再將免疫清除組26例感染者按ALT高低分為兩個(gè)組(80 U/L,17例,(4.06±1.05)%和80 U/L,9例,(3.87±0.93)%),分析顯示同樣的結(jié)果,在兩組中均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。 2.在CD4+CD25+CD127low T細(xì)胞占CD4+ T細(xì)胞的百分比中,免疫耐受組(4.91±0.92)%和免疫清除組(4.94±1.34)%的比例均高于非活動(dòng)或低(非)復(fù)制組(4.19±0.86)%,差異均具有統(tǒng)計(jì)學(xué)意義(t=3.251,P=0.002和t=2.558,P=0.013);且病毒陽(yáng)性組(免疫耐受組+免疫清除組,HBeAg陽(yáng)性,(4.93±1.11)% )的比例高于病毒陰性組(非活動(dòng)或低(非)復(fù)制組,HBeAg陰性,(4.19±0.86)%)(t=3.208,P=0.002),差異具有統(tǒng)計(jì)學(xué)意義。將ALT異常組(免疫清除組)與ALT正常組(免疫耐受組+非活動(dòng)或低(非)復(fù)制組)的比例進(jìn)行比較,結(jié)果顯示兩組的差異(4.94±1.34)%與(4.57±0.95)%之間無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);再將免疫清除組26例感染者按ALT高低分為兩個(gè)組(80 U/L,17例,(4.93±1.48)%和80 U/L,9例,(4.98±1.11)%),分析顯示同樣的結(jié)果,在兩組中均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3.將CD4+CD25+Foxp3+和CD4+CD25+CD127low檢測(cè)的所有感染者的細(xì)胞頻率進(jìn)行比較,結(jié)果顯示在兩種表型檢測(cè)的結(jié)果均值中前者的頻率(3.95±0.88)%明顯低于后者(4.68±1.08)%(Z=-4.718,P0.001),即用CD127low檢測(cè)CD4+CD25+ T細(xì)胞的結(jié)果明顯高于用Foxp3+檢測(cè)的結(jié)果,兩者之間的差異具有統(tǒng)計(jì)學(xué)意義。 結(jié)論: CD4+CD25+調(diào)節(jié)性T細(xì)胞在慢性HBV感染者病毒高復(fù)制組中明顯升高,并與感染者ALT的高低無(wú)明顯關(guān)系。CD4+CD25+ T細(xì)胞中CD127分子的低表達(dá)與Foxp3轉(zhuǎn)錄因子的表達(dá)有一致性,但前者代表Treg的頻率明顯高于后者。
[Abstract]:Objective: to investigate the level of CD4 CD25 Foxp3 regulatory T cells in peripheral blood of patients with chronic HBV infection, and to analyze the role of Treg in the mechanism of chronic HBV infection and its correlation with the progression of HBV infection. The relationship between the expression of Foxp3 transcription factor and its two cell markers-CD127 was studied. Methods: peripheral whole blood samples were collected from 34 patients with chronic HBV infection in immune tolerance phase (n = 26) and chronic HBV infection (n = 31) in inactive or low (low) replication stage. Flow cytometry (FCM) was used. By fluorescent staining on the surface and internal markers of CD4, CD25, Foxp3 and CD127, the cell population was set up by flow cytometry, and the frequencies of CD4 CD25 Foxp3 T cells and CD4 CD25 CD127 low T cells were detected. Results: 1. In the percentage of CD4 CD25 Foxp3 T cells to CD4 T cells, The percentage of immune tolerance group (4.17 鹵0.75)% was higher than that of inactive or low (non-replicating) group (3.66 鹵0.85), the difference was statistically significant (ZAI-2.693P0. 007), and the proportion of virus positive group (immune tolerance group: 60 cases of HBeAg positive group, (4.09 鹵0.86)%) was higher than that of virus negative group (non-live group). There were 31 cases (3.66 鹵0.85) of HBeAg negative in motility or low (non-replicating) group (t = 2.266) (P < 0.026). The difference was statistically significant. The ratio of alt abnormal group (immune clearance group) and normal alt group (65 cases of inactive or low replication group) was compared. The results showed that there was no significant difference between the two groups (3.99 鹵1.00)% and (3.93 鹵0.83)% (P0.05), and 26 cases of infected persons in immune clearance group were divided into two groups according to alt level (80 UL / L 17 cases, (4.06 鹵1.05)% and 80 UL / P 9 cases, (3.87 鹵0.93)%). There was no statistical significance in both groups (P0.05). In the percentage of CD4 CD25 CD127low T cells to CD4 T cells, The percentage of immune tolerance group (4.91 鹵0.92)% and immune clearance group (4.94 鹵1.34)% was higher than that of inactive or low replication group (4.19 鹵0.86), the difference was statistically significant (t _ (3.251) P _ (0.002) and t _ (2.558) P _ (0.013), and the proportion of virus positive group (immune tolerance group) was (4.93 鹵1.11)%. HBeAg was significantly higher than that in viral negative group (inactive or low replication group, (4.19 鹵0.86)%) (t = 3.208, P < 0.002). The difference was statistically significant. The ratio of alt abnormal group (immune clearance group) and normal alt group (immune tolerance group inactive or low (non-replicating) group) was compared. The results showed that there was no significant difference between the two groups (4.94 鹵1.34)% and (4.57 鹵0.95)% (P0.05), and 26 cases of infected persons in the immune clearance group were divided into two groups according to the alt level (80 UL / L 17 cases, (4.93 鹵1.48)% and 80 U L / P 9 cases, (4.98 鹵1.11)%). The cell frequencies of all infected individuals detected by CD4 CD25 Foxp3 and CD4 CD25 CD127low were compared. The results showed that the frequency of the former (3.95 鹵0.88)% was significantly lower than that of the latter (4.68 鹵1.08)% (ZP0.001), that is, the CD4 CD25T cell detected by CD127low was significantly higher than that by Foxp3, and the difference between them was statistically significant. Conclusion: CD4 CD25 regulatory T cells were significantly increased in chronic HBV infected patients with high replication. The low expression of CD127 in CD4 CD25 T cells was consistent with the expression of Foxp3 transcription factor, but the frequency of Treg in the former was significantly higher than that in the latter.
