【摘要】： 臂叢鎖骨下部神經(jīng)解剖關(guān)系復(fù)雜,是臨床上肢神經(jīng)阻滯、臂叢神經(jīng)損傷的重要內(nèi)容。本研究通過對臂叢鎖骨下部神經(jīng)的解剖,進(jìn)一步熟悉和掌握此部分的組成及其內(nèi)部臂叢的類型、走行、分布和與周圍組織的位置關(guān)系,并進(jìn)行解剖測量,為臨床手術(shù)及麻醉提供參考。 研究發(fā)現(xiàn),鎖骨中點(diǎn)下緣及喙突下神經(jīng)較集中,可作為上臂神經(jīng)阻滯的進(jìn)針點(diǎn),可于鎖骨中點(diǎn)下約2cm處進(jìn)針,或于喙突下一橫指處進(jìn)針,將取得較好的神經(jīng)阻滯效果。臂叢鎖骨下部神經(jīng)位于腋鞘內(nèi),與腋血管關(guān)系密切,此處可能有聯(lián)合血管神經(jīng)損傷,在腋窩、鎖骨下手術(shù)路入時也應(yīng)注意其相互關(guān)系。 鎖骨下肌、肋鎖韌帶、胸小肌下間隙與臂叢鎖骨下部神經(jīng)關(guān)系密切,相關(guān)的病變將有可能造成臂叢神經(jīng)鞘的卡壓,引起胸廓出口綜合征的臨床癥狀。
[Abstract]:The anatomical relationship of the lower clavicular nerve of brachial plexus is complex, which is an important content of clinical upper limb nerve block and brachial plexus nerve injury. Based on the anatomy of the lower clavicular nerve of brachial plexus, this study is to know and master the composition of this part and the types of internal brachial plexus, walk, distribute and position relation with the surrounding tissues, and make anatomic measurement. To provide reference for clinical operation and anesthesia. It is found that the inferior edge of the middle clavicle and the subcoracoid nerve are concentrated and can be used as the needle point of the upper brachial nerve block. It can be used at about 2cm under the middle point of the clavicle or at the transverse finger under the coracoid process to obtain a better nerve block effect. The inferior clavicular nerve of brachial plexus is located in axillary sheath and is closely related to axillary vessels. The subclavian muscle, intercostal clavicular ligament, subpectoralis space and the lower clavicular nerve of brachial plexus are closely related to each other. The related lesions may result in the entrapment of brachial plexus sheath and the clinical symptoms of thoracic outlet syndrome.
