【摘要】： 目的:近幾年對(duì)斜坡區(qū)手術(shù)入路的研究是顱底外科和臨床解剖的熱點(diǎn),該區(qū)域病變的手術(shù)一直被認(rèn)為是神經(jīng)外科的難題,其手術(shù)死亡率和傷殘率高。該區(qū)域的骨性解剖數(shù)據(jù)(尤其是中國(guó)北方人)還不全面。為了給斜坡區(qū)的手術(shù)入路的選擇和安全手術(shù)范圍的制定提供準(zhǔn)確的解剖學(xué)依據(jù),我們將較全面地測(cè)量斜坡區(qū)骨性結(jié)構(gòu)相關(guān)解剖數(shù)據(jù)。材料與方法:本研究通過(guò)對(duì)顱的水平切面10個(gè)標(biāo)本和顱的正中矢狀切面10個(gè)標(biāo)本進(jìn)行測(cè)量,得出了切牙孔、前鼻棘、后鼻棘、枕骨大孔前端、枕髁前端、卵圓孔、破裂孔、頸動(dòng)脈管外口及舌下神經(jīng)管外口的內(nèi)側(cè)緣至咽結(jié)節(jié)及后四項(xiàng)至中線的距離;斜坡顱底面高度;枕骨大孔縱徑(FML)、枕骨大孔前正中點(diǎn)與枕髁后緣連線垂直距離(AOCP)、枕髁軸徑(OCA)、枕髁間距。并且把這些數(shù)據(jù)與同類研究的結(jié)果進(jìn)行比較。結(jié)論:顱底手術(shù)基本原則為普遍適用。參照本文測(cè)量得到的破裂孔至咽結(jié)節(jié)距離和斜坡顱底面高度,對(duì)斜坡區(qū)手術(shù)時(shí)開骨窗的安全范圍給出了一些新結(jié)論,所以開骨窗的實(shí)際操作要根據(jù)地理位置和人種做適當(dāng)?shù)恼{(diào)整。
[Abstract]:Objective: in recent years, the study of surgical approach to Clivus region is a hot spot in skull base surgery and clinical anatomy. The operation of this area is always regarded as a difficult problem in neurosurgery, and the operative mortality and disability rate are high. The data on bone anatomy in the region, especially in northern China, are incomplete. In order to provide an accurate anatomical basis for the choice of surgical approach and the formulation of safe surgical scope in the clival region, we will comprehensively measure the anatomical data related to the bone structure of the clival region. Materials and methods: the foramen incisor anterior nasal spine posterior nasal spine occipital foramen anterior occipital foramen occipital condyle anterior occipital foramen occipital condyle foramen ovale and rupture foramen were measured. The distance from the medial margin of the external orifice of the carotid artery and the hypoglossal canal to the pharyngeal nodule and the posterior four to the midline, the height of the base of the Clivus skull; The longitudinal diameter of foramen magnum of occipital bone and the vertical distance between the anterior midpoint of occipital foramen and the posterior edge of occipital condyle were observed and the distance between occipital condyle and occipital condyle was observed. These data were compared with the results of similar studies. Conclusion: the basic principle of skull base operation is universal. With reference to the distance from ruptured foramen to pharyngeal nodule and the height of the base of the Clivus, some new conclusions are given on the safe range of the open bone window during the operation in the clival region. Therefore, the actual operation of bone window should be adjusted according to geographical location and race.
