【摘要】： 目的:通過對(duì)育齡婦女月經(jīng)不調(diào)患者的情緒狀態(tài)分析,研究育齡婦女月經(jīng)不調(diào)與情緒狀態(tài)之間的關(guān)系及其影響因素和癥狀特點(diǎn),初步探索育齡婦女月經(jīng)不調(diào)與中醫(yī)肝經(jīng)證候之間的關(guān)聯(lián)性。 對(duì)象方法:研究對(duì)象為2007年8月至2007年12月在成都中醫(yī)藥大學(xué)附屬醫(yī)院婦科就診的18歲～40歲的女性人群。以育齡期婦女月經(jīng)不調(diào)與非月經(jīng)不調(diào)的納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)為依據(jù)選擇被調(diào)查者,以“育齡婦女月經(jīng)情況調(diào)查表”、“中醫(yī)肝臟象情緒量表(ERSG)”為調(diào)查工具,采用流行病學(xué)橫段面調(diào)查法,對(duì)符合納入標(biāo)準(zhǔn)的被調(diào)查者進(jìn)行問卷調(diào)查。借助SPSS13.0統(tǒng)計(jì)軟件包,采用頻數(shù)分析、軼和分析、方差分析、Logistic回歸分析對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)。 結(jié)果: 1.育齡婦女月經(jīng)不調(diào)者與非月經(jīng)不調(diào)者在教育程度、婚姻狀況、人工流產(chǎn)情況構(gòu)成比上有顯著的差異性。 2.育齡婦女月經(jīng)不調(diào)者和非月經(jīng)不調(diào)者情緒狀態(tài)間有顯著差異性。育齡婦女月經(jīng)不調(diào)者的情緒狀態(tài)以容易緊張、愛發(fā)脾氣、乳房脹痛為主要癥狀。 3.不同月經(jīng)病況的育齡婦女,其情緒狀態(tài)有所不同。月經(jīng)先期,月經(jīng)量多和月經(jīng)先后不定期患者以肝經(jīng)情緒和焦慮情緒為主,而月經(jīng)后期、月經(jīng)量少患者則主要表現(xiàn)為抑郁情緒。 結(jié)論:育齡婦女月經(jīng)不調(diào)患者存在顯著的情緒異常,若對(duì)其進(jìn)行相應(yīng)的情志調(diào)節(jié),可緩解心理壓力,減少月經(jīng)不調(diào)的發(fā)病,這對(duì)改善生活質(zhì)量,提高生殖健康有積極的意義。此外月經(jīng)不調(diào)患者普遍存在肝經(jīng)癥狀,若從“肝主疏泄,調(diào)節(jié)情志”的角度進(jìn)行中醫(yī)藥干預(yù)治療,可能對(duì)月經(jīng)不調(diào)患者的治療有重要意義。
[Abstract]:Objective: to study the relationship between menstrual disorder and emotional state in women of childbearing age by analyzing their emotional state, and to study the influencing factors and symptom characteristics of menstrual disorder in women of childbearing age. To explore the relationship between irregular menstruation of women of childbearing age and syndrome of liver meridian of traditional Chinese medicine. Object: from August 2007 to December 2007, women aged from 18 to 40 years old were admitted to gynecology department of affiliated Hospital of Chengdu University of traditional Chinese Medicine (Chengdu University of traditional Chinese Medicine). According to the inclusion and exclusion criteria of menstrual and non-menstrual disorders of women of childbearing age, the questionnaire of menstruation of women of childbearing age and the scale of TCM liver mood were used as the investigation tools. An epidemiological cross-sectional survey was used to investigate the subjects who met the inclusion criteria. With the help of SPSS13.0 statistical software package, the data were analyzed by frequency analysis, analysis of Yi sum, and logistic regression analysis of variance. Results: 1. There were significant differences in educational level, marital status and the constitution of induced abortion between women of childbearing age with irregular menstruation and those with irregular menstruation. 2. There were significant differences in emotional state between menstrual disorders and non-menstrual disorders among women of childbearing age. The emotional state of women of childbearing age is characterized by tenseness, temper and breast pain. 3. Different menstrual conditions of women of childbearing age, their emotional state is different. The patients with premenstrual period, more menstrual volume and irregular menstruation were mainly in the mood of liver meridian and anxiety, but in the late period of menstruation, the patients with less menstrual volume mainly showed depression. Conclusion: there are significant emotional abnormalities in women of childbearing age with irregular menstruation. If they are adjusted, the psychological pressure can be alleviated and the incidence of irregular menstruation can be reduced, which has a positive significance in improving the quality of life and improving reproductive health. In addition, patients with irregular menstruation generally have symptoms of liver meridian. It may be of great significance to treat patients with irregular menstruation by Chinese medicine intervention treatment from the angle of "liver control and catharsis, regulating emotion".
