【摘要】： 目的:1、調查接受輔助生殖技術治療的不孕女性的心理社會特點包括生育觀念、情緒、個性特點、社會支持狀況、應付方式等方面,探討影響其情緒的相關因素。2、調查體外受精女性在不同治療階段的情緒特點以及心理壓力對其治療結果的影響。 研究方法:試驗組:使用自編的一般情況調查表和生育相關問卷、焦慮自評問卷(SAS)、貝克抑郁問卷(BDI)、艾森克個性問卷(EPQ)、應付方式問卷(CSQ)、社會支持評定量表(SSRS)對接受輔助生殖技術治療的217例不孕女性進行調查,根據不孕原因分為女性原因組、男性原因組、雙方原因組,比較各組之間心理學變量的差異,分析影響患者情緒的相關因素。對其中首次接受體外受精-胚胎移植(IVF-ET)的女性分別在降調當日(T1)、注射HCG當日(T2)、移植前1天(T3)、移植后第14天(T4)等四個時間點,由患者使用SAS、BDI對其前一周內的情緒進行自我測評。根據移植后第14天血β-HCG及第28天B超結果,把IVF-ET女性分成妊娠組和非妊娠組。比較各組之間的情緒差別及不同治療階段情緒變化特點。對照組:使用自編的一般情況調查表和生育相關問卷、SAS、BDI、EPQ、CSQ、SSRS對某社區(qū)生育力正常的已婚育齡婦女進行調查,共回收問卷52份。 結果:1、輔助生育技術成為大部分不孕患者最后的選擇,傳統(tǒng)生育觀念在不孕人群中仍占有一定的比例。2、不孕各組焦慮情緒評分均高于對照組;女性原因組抑郁評分高于其他各組。女性原因組人格特點傾向于固執(zhí)、內向、多疑、情緒不穩(wěn)等,男性原因組和雙方原因組人格特點和對照組無差異。各組不孕女性獲得的社會支持低于對照組。女性原因組更傾向于自責和幻想,較少求助;男性原因組采韌吮堋⒑俠砘彡撓Χ苑絞繳儆詼哉兆欏?、經多元線性回歸分析,對焦慮情緒影響較大的因素有神經質、經濟壓力、解決問題、文化程度、精神質、求助。對抑郁情緒影響較大的因素有自責、經濟壓力、神經質、求助、幻想、精神質。4、通過對IVF女性情緒特點的縱向調查發(fā)現(xiàn),妊娠組、非妊娠組和對照組相比,T1、T2、T4時,IVF女性焦慮評分高于對照組,T1時,IVF女性抑郁評分高于對照組,T4時,妊娠組抑郁情緒和對照組無差異,非妊娠組高于對照組。妊娠組和非妊娠組相比,T1、T2、T3四個時期兩組之間焦慮和抑郁評分無統(tǒng)計學差異;T4時期,非妊娠組焦慮和抑郁分數(shù)均顯著高于妊娠組。 結論:1、和正常生育女性相比,不孕女性承擔很大的心理壓力,其中女性原因者壓力最大。2、心理壓力受到個性特點、應對方式、社會支持、經濟壓力、文化程度等多種因素的影響。3、在接受IVF治療的不同階段,不孕女性所面臨的壓力源是不同的。因此,可以根據治療的不同階段,實施有針對性的干預措施。4、不能認為心理壓力會影響IVF治療結局,但是失敗的治療結果會引起IVF女性焦慮和抑郁情緒顯著升高。
[Abstract]:Objective: 1. To investigate the psycho-social characteristics of infertile women treated with assisted reproductive technology, including fertility concept, mood, personality, social support, coping style, etc.To investigate the emotional characteristics of IVF women in different stages of treatment and the influence of psychological stress on the outcome of treatment.Research methods: test group: use self-designed general information questionnaire and fertility related questionnaire,Self-rating anxiety questionnaire (SAS), Beck Depression questionnaire (BDI), Eysenck Personality questionnaire (EPQ), coping style questionnaire (CSQ) and Social support rating scale (SSRS) were used to investigate 217 infertile women who were treated with assisted reproductive technology. According to the causes of infertility, they were divided into female cause group.The differences of psychological variables between male cause group and bilateral cause group were compared, and the related factors affecting patients' emotion were analyzed.The first IVF-ETs of IVF-ETs of IVF-ETs were used to assess the emotion of the patients in the previous week. The first time points were T1D, the day of HCG injection, the day before transplantation, the day before transplantation, and the day 14 after transplantation.According to the results of blood 尾-HCG and B-ultrasound on the 14th day after transplantation, IVF-ET women were divided into pregnancy group and non-pregnancy group.The differences of emotion and the characteristics of emotional changes in different treatment stages were compared.Control group: a total of 52 questionnaires were collected from married women of childbearing age who had normal fertility in a community.Results: 1. Assisted fertility technology was the last choice for most infertile patients. The traditional conception of fertility still occupied a certain proportion in infertile population. The scores of anxiety in infertile group were higher than those in control group.The depression score of female cause group was higher than that of other groups.The personality characteristics of female cause group were obstinate, introverted, paranoid, emotional instability, and there was no difference between male cause group and bilateral cause group and control group.The social support of infertile women in each group was lower than that in the control group.The female cause group was more inclined to blame itself and fantasize, and the male reason group took the tough sucking and sucking. By multiple linear regression analysis, the factors affecting anxiety were neuroticism and economic pressure.Problem solving, education level, mental quality, help.The influential factors of depression were self-reproach, economic stress, neuroticism, seeking help, fantasy, psychoticism. The longitudinal investigation of emotional characteristics of IVF women showed that the pregnancy group,Compared with the control group, the anxiety scores of the IVF women in the non-pregnant group were higher than those in the control group at T _ 1 and T _ 2 and T _ 4, and the depression scores in the non-pregnancy group were higher than those in the control group, but there was no difference between the pregnancy group and the control group, and the non-pregnancy group was higher than that in the control group.There was no significant difference in anxiety and depression scores between the two groups in the four stages of T _ 1T _ 2T _ 3. The scores of anxiety and depression in the non-pregnancy group were significantly higher than those in the pregnancy group.Conclusion: compared with normal women, infertile women bear great psychological pressure. Among them, female causes are the most stressful, and psychological pressure is influenced by personality, coping style, social support, economic pressure.Education and other factors. 3, in the different stages of IVF treatment, infertile women are faced with different sources of stress.Therefore, according to the different stages of treatment, we can carry out targeted intervention measures. 4. We can not think that psychological pressure will affect the outcome of IVF treatment, but the failure of treatment results will cause anxiety and depression of IVF women significantly increased.
