健康人不同睡眠時相心率變異性的差異

【摘要】:目的比較健康人不同睡眠時相心率變異性(HRV)的差異。方法試驗數(shù)據(jù)取自Sleep Heart Rate and Stroke Volume Data Bank,包含45例健康人睡眠期竇性心率RR間期序列(RRI)和睡眠時相數(shù)據(jù)。RRI序列從采樣率為500 Hz的心電數(shù)據(jù)中提取,睡眠時相數(shù)據(jù)分為醒覺期(WAKE)、快速動眼期(REM)及非快速動眼期(NREM)3個睡眠時相。將每一位受試者睡眠期RRI序列分成以5 min為單位的時間片,計算各單位時間片的HRV指標,其中頻域法指標包括極低頻能量(VLF)、低頻能量(LF)、高頻能量(HF)、總能量(TP)及低頻與高頻能量比(LF/HF);時域法指標包括5 min平均心率(HR)、正常-正常RR間期標準差(SDNN)、相鄰RR間期差值均方根(RMSSD)、相鄰RR間期差值大于50 ms的百分數(shù)(pNN50)及RR間期變化率(RRIV)。將睡眠時相數(shù)據(jù)序列分成同步的單位時間片,并標記各單位時間片內包含的睡眠時相數(shù)。結果研究對象共包含3 952個單位時間片,共納入符合條件的單位時間片1 477個,其中,WAKE期240個(16.2%),REM期233個(15.8%),NREM期1 004個(68.0%)。不同睡眠時相頻域法HRV指標比較,差異均有統(tǒng)計學意義(P0.05);其中NREM期VLF、LF、TP、LF/HF與WAKE期比較,差異均有統(tǒng)計學意義(P0.05);NREM期VLF、LF、HF、TP、LF/HF與REM期比較,差異均有統(tǒng)計學意義(P0.05);REM期HF與WAKE期比較,差異有統(tǒng)計學意義(P0.05)。不同睡眠時相時域法HRV指標比較,差異均有統(tǒng)計學意義(P0.05);其中NREM期HR、SDNN、RRIV與WAKE期比較,差異均有統(tǒng)計學意義(P0.05);NREM期HR、SDNN、RRIV、RMSSD、pNN50與REM期比較,差異均有統(tǒng)計學意義(P0.05);REM期RMSSD、pNN50與WAKE期比較,差異均有統(tǒng)計學意義(P0.05)。Spearman秩相關分析結果顯示,RMSSD、pNN50與HF呈正相關(rs=0.95、0.94,P0.001);RRIV與LF呈正相關(rs=0.79,P0.001);SDNN與TP呈正相關(rs=0.98,P0.001);RMSSD與pNN50呈正相關(rs=0.98,P0.001)。結論 NREM期副交感神經(jīng)興奮性高,REM期交感神經(jīng)興奮性占主導,時域法和頻域法指標均能反映不同睡眠時相HRV的差異。
[Abstract]:Objective to compare heart rate variability (HRV) of different sleep phases in healthy people.Methods the experimental data were obtained from Sleep Heart Rate and Stroke Volume Data Bank.RRI data were extracted from ECG data of 500Hz sampling rate in 45 healthy subjects, including sinus heart rate RR interval sequence (RRI) and sleep phase data (RRI).The sleep phase data were divided into three sleep phases: wakeup, REM (REM) and NREM (non-REM).The RRI sequence of each subject during sleep was divided into 5 min time slices, and the HRV index of each unit time slice was calculated.The parameters of frequency domain include very low frequency energy (VLF), low frequency energy (LFN), high frequency energy (HF), total energy (total energy) and low frequency / high frequency energy ratio (LFR / HFN), and time domain method includes 5 min mean heart rate (HR), normal normal RR interval standard deviation (SDN), adjacent RR interval.RMSSD, the percentage of adjacent RR interval greater than 50 Ms (pNN50) and RR interval change rate were higher than RRIVP.The sequence of sleep phase data is divided into synchronized unit time slices and the number of sleep phases included in each unit time slice is marked.Results A total of 3 952 unit time slices were included in the study, and 1 477 unit time slices were included, of which 240 were in WAKE phase and 233 were in REM stage. 1 004 units were included in NREM phase.涓嶅悓鐫$湢鏃剁浉棰戝煙娉旽RV鎸囨爣姣旇緝,宸紓鍧囨湁緇熻瀛︽剰涔,
