重慶市主城區(qū)護(hù)理人員職業(yè)緊張與心理健康影響因素的相關(guān)研究

【摘要】: 職業(yè)緊張對(duì)健康、行為以及對(duì)工作效率的影響已成為國際上重要的職業(yè)衛(wèi)生問題之一。職業(yè)緊張的研究已成為現(xiàn)代社會(huì)重要的公共衛(wèi)生問題,已受到越來越多的社會(huì)學(xué)家、生理學(xué)家、心理學(xué)家以及政府部門的關(guān)注。護(hù)理人員是為人們提供健康服務(wù)的特殊群體,其職業(yè)緊張狀況影響著護(hù)理人員心理健康,也影響著她們提供的護(hù)理質(zhì)量水平。因此關(guān)于護(hù)理人員的職業(yè)緊張研究也越來越受到國內(nèi)外學(xué)者的關(guān)注。 本研究旨在探討重慶市護(hù)理人員的職業(yè)緊張、心理健康狀況以及影響職業(yè)緊張和心理健康的相關(guān)因素。本研究通過問卷調(diào)查的方式,采用癥狀自評(píng)量表(SCL-90),職業(yè)緊張問卷(OSI-R)和卡特爾16項(xiàng)人格因素(16-PF)對(duì)740名護(hù)理人員的職業(yè)緊張及心理健康狀況以及它們的相關(guān)因素進(jìn)行了調(diào)查。并采用t檢驗(yàn)、F檢驗(yàn)等數(shù)據(jù)分析方法,以工作年限、用工性質(zhì)、婚姻狀況等為變量,對(duì)心理健康水平、職業(yè)緊張狀況以及相關(guān)因素作了差異分析。其結(jié)果如下: 1、護(hù)士職業(yè)緊張現(xiàn)狀及影響因素 與成渝兩地專業(yè)技術(shù)人員常模比較,重慶市護(hù)理人員職業(yè)緊張情況為:職業(yè)任務(wù)總均分的差異無顯著(P㧐0.05);但個(gè)體緊張反應(yīng)總均分明顯高于常模值,個(gè)體應(yīng)對(duì)資源總均分明顯低于常模值,差異有顯著性(P㩳0.01) 職業(yè)緊張狀況的影響因素包括工齡、婚姻狀況、文化程度、用工性質(zhì)等人口學(xué)特征以及護(hù)理人員的人格特質(zhì)等。個(gè)體應(yīng)對(duì)資源得分最高的護(hù)理人員工作年限在20年以上,工作年限10-15年的護(hù)理人員,應(yīng)對(duì)資源最少(P0.01);離異及喪偶者的職業(yè)任務(wù)得分高于已婚者,已婚高于未婚(P0.01).不同的人格特質(zhì)對(duì)職業(yè)緊張?jiān)吹捏w驗(yàn)具有差異性,個(gè)體緊張反應(yīng)及應(yīng)對(duì)資源也不同。 2、心理健康現(xiàn)狀及影響因素 與全國普通人常模的比較,重慶市護(hù)理人員SCL-90總分高于全國常模,且差異顯著。 心理健康的影響因素包括婚姻狀況、用工性質(zhì)等人口學(xué)特征、職業(yè)任務(wù)情況、個(gè)體應(yīng)對(duì)資源以及人格特質(zhì)等。離異或喪偶者SCL-90的總均分以及軀體化、強(qiáng)迫、人際敏感、焦慮、恐怖、精神病性緯度的得分明顯高于未婚及已婚者,且差異有顯著性( p0.05);護(hù)理人員的SCL-90總均分與職業(yè)任務(wù)、緊張反應(yīng)各子項(xiàng)均呈明顯正相關(guān)(P0.01),與個(gè)體應(yīng)對(duì)資源各子項(xiàng)均呈明顯負(fù)相關(guān)(P0.01);以SCL-90總均分為應(yīng)變量,護(hù)士各人格特質(zhì)為自變量,進(jìn)行多元線性逐步回歸分析,顯示影響護(hù)士心理健康的主要人格特質(zhì)(從強(qiáng)到弱)為緊張性、憂慮性、自律性、穩(wěn)定性、敏感性、創(chuàng)造性、樂群性、懷疑性、有恒性。 綜上所述,護(hù)理人員的職業(yè)緊張水平較高,心理健康狀況不容樂觀。影響護(hù)理人員的職業(yè)緊張及心理健康狀況的因素是多方面的。可以根據(jù)不同人口學(xué)特征、人格情況等,提出有針對(duì)性的緩解職業(yè)緊張的措施;明確護(hù)士工作的責(zé)任、提高應(yīng)對(duì)能力、培養(yǎng)良好的護(hù)士職業(yè)角色人格等是提高護(hù)理人員心理健康水平的有效途徑。
[Abstract]:Occupation stress on health behavior, and the influence on the efficiency of the occupation health problem has become one of the important international research. The occupation stress has become an important public health problem in modern society, has been more and more sociologists, psychologists, physiologists, and government departments concerned. The nursing staff is a special group to provide health services for the people the occupation, tense situation affects the mental health of nursing staff, but also affect the quality of care they provide. So the level of occupation tension research on nursing staff is also more and more domestic and foreign scholars.
The purpose of this study was to investigate the nursing staff of Chongqing city occupation tension, mental health and occupation stress and mental health related factors. This research through the questionnaire survey, using the symptom checklist (SCL-90), occupation stress questionnaire (OSI-R) and Cattell 16 personality factors (16-PF) occupation stress and mental health of the 740 nurses and their related factors were investigated. By t test, F test and other methods of data analysis, to work, the nature of employment, marital status variables on mental health level, the difference analysis of occupation stress state and related factors. The results are as follows:
1, the status and influencing factors of occupational stress of nurses
Compared with the Chengdu and Chongqing professional technical personnel norm, Chongqing City nursing staff occupation tension: the difference between total average occupation tasks significantly (P? 0.05); but the stress response of the individual total were significantly higher than the normal value, the total average personal resources was significantly lower than the normal value, there was a significant difference (P? 0.01)
Influence factors of occupation tension include age, marital status, education level, character of employment and the nursing staff's personality traits. The coping resources score the highest working life of nursing staff in more than 20 years, the nursing staff working life of 10-15 years, with the least resources (P0.01); occupation task scores of divorced and widowed the higher than married, married than unmarried (P0.01). The experience of different personality traits on occupation tension source has the difference, the individual stress response and coping resources are also different.
2, the status of mental health and its influencing factors
Compared with the common norm in China, the total score of SCL-90 nursing staff in Chongqing was higher than that of the national norm, and the difference was significant.
Mental health factors include marital status, the character of employment, occupation tasks, personal resources and personality traits. The total average divorced or widowed SCL-90, somatization, compulsion, interpersonal sensitivity, anxiety, terror, scores of psychosis was significantly higher than that of weft unmarried and married, have significant the difference (P0.05); the total scores of SCL-90 and the occupation task of nursing staff, stress response sub items were positively correlated (P0.01), and the personal resources are negatively correlated with the total score (P0.01); SCL-90 as the dependent variable, the nurses' personality traits as independent variables, multiple linear regression analysis showed the main effects of personality traits, psychological health of nurses (from strong to weak) for tension, anxiety, self-discipline, stability, sensitivity, creativity, sociability, vigilance, perseverance.
To sum up, the level of nursing staff occupation tension high, mental health is not optimistic. The factors influencing occupation stress and mental health of nursing staffs are in many aspects. According to the different demographic characteristics, personality, etc., are put forward to alleviate the tension of occupation measures; clear the responsibility of nurses, improve the ability to deal with the cultivation of good nurse occupation role personality is an effective way to improve the mental health level of nurses.
