骨髓間充質干細胞關節(jié)腔注射治療兔早期類風濕性關節(jié)炎的實驗研究

【摘要】:目的:用骨髓間充質干細胞關節(jié)腔注射治療兔早期類風濕性關節(jié)炎,探討其可行性及療效評價。 方法:將60只新西蘭大白兔隨機分成對照組、類風濕性關節(jié)炎組和骨髓間充質干細胞治療組,每組20只。治療組在第8周右膝關節(jié)腔內注射骨髓間充質干細胞。觀察動物的一般情況,TNF-α和IL-1β水平,MRI普通以及增強掃描右膝關節(jié),取膝關節(jié)標本行大體及病理觀察。 結果:所有兔子的一般情況尚好,與類風濕性關節(jié)炎組相比,治療后骨髓間充質干細胞治療組關節(jié)腫脹程度減輕明顯,步態(tài)漸好轉;血清TNF-α和IL-1β水平降低;MRI示低信號區(qū)縮小,滑膜的厚度減小,關節(jié)軟骨厚度接近正常,無明顯關節(jié)腔積液;大體標本見滑膜有輕度炎性增生,較類風濕性關節(jié)炎組輕,關節(jié)軟骨表面有新生軟骨覆蓋,呈乳白色,關節(jié)面基本平整;病理檢查提示有透明樣軟骨形成,細胞排列整齊,較正常略厚,與軟骨相連,,關節(jié)面基本平整;滑膜炎癥減輕,炎性細胞明顯較少。 結論:骨髓間充質干細胞在關節(jié)腔內可向軟骨分化,抑制關節(jié)滑膜炎癥,為早期類風濕性關節(jié)炎患者提供了一種新的有效療法。
[Abstract]:Objective: to evaluate the feasibility and efficacy of bone marrow mesenchymal stem cell injection in the treatment of early rheumatoid arthritis in rabbits. Methods: sixty New Zealand rabbits were randomly divided into control group, rheumatoid arthritis group and bone marrow mesenchymal stem cell treatment group. 20 rats in each group. Bone marrow mesenchymal stem cells were injected into the right knee joint cavity in the treatment group at the 8th week. The general situation of the animals was observed and the levels of TNF- 偽 and IL-1 尾 were compared with conventional MRI and enhanced scanning of the right knee. The specimens of knee joint were taken for gross and pathological observation. Results: the general condition of all rabbits was good. Compared with rheumatoid arthritis group, bone marrow mesenchymal stem cell treatment group reduced the degree of joint swelling obviously, and the gait gradually improved. The serum levels of TNF- 偽 and IL-1 尾 were decreased. MRI showed that the low signal area was reduced, the thickness of synovial membrane was decreased, the thickness of articular cartilage was close to normal, and there was no obvious effusion in articular cavity. Gross specimens showed mild inflammatory hyperplasia of synovium, lighter than that of rheumatoid arthritis group, the articular cartilage surface was covered with new cartilage, it was milky white, and the articular surface was flat. Pathological examination showed that hyalurondroid cartilage was formed, cells were arranged neatly, slightly thicker than normal, connected with cartilage, and the articular surface was flat. Synovitis is alleviated, inflammatory cell is obviously less. Conclusion: bone marrow mesenchymal stem cells can differentiate into cartilage in articular cavity, inhibit synovitis, and provide a new effective therapy for early rheumatoid arthritis patients.
