人腦膠質瘤中CD133、SSEA-1和Nestin的表達和意義

【摘要】:目的:膠質瘤是腦腫瘤中最常見的腫瘤,約占50%。盡管目前的外科手術,,放、化療都有了極大的提高,但是對于膠質瘤的治療及預后,還遠未達到到理想的效果。膠質瘤干細胞的提出,給我們提供了一個全新的思路。本實驗欲研究膠質瘤干細胞標志物CD133、SSEA-1(階段特異性胚胎抗原-1)和Nestin(神經巢蛋白)在人腦膠質瘤組織中表達及與病理分級的相關性;探討三者在人腦膠質瘤的診斷及惡性程度判斷中的臨床意義,以及為治療提供分子生物學理論基礎。 方法:收集廣西醫(yī)科大學一附院(2011年9月—2012年12月)經病理證實為膠質瘤,按2007年WHO分級標準:Ⅰ級6例,Ⅱ級16例,Ⅲ級13例,Ⅳ級19例,另外用6例正常腦組織為對照。應用免疫組織化學的方法檢測54例腦膠質瘤組織和6例正常腦組織標本中CD133、SSEA-1及Nestin的表達。 結果:①54例腦膠質瘤組織和6例正常腦組織標本中CD133陽性細胞率率分別為25.38%和0%(P0.05),SSEA-1陽性細胞率分別為26.62%和0%,Nestin陽性細胞率分別為22.39%和0%。②對54例腦膠質瘤組織中CD133和SSEA-1及Nestin陽性細胞率分別進行多個病理級別之間的均數比較: I級和Ⅱ級之間差別無統計學意義(P>0.05),Ⅰ級和Ⅲ、Ⅳ級間差別具有統計學意義(P<0.01),Ⅱ級和Ⅲ、Ⅳ級間差別具有統計學意義(P<0.05), Ⅲ級和Ⅳ級間差別具有統計學意義(P<0.05)。③腦膠質瘤組織中SSEA-1陽性細胞率大于CD133大于Nestin,差別具有統計學意義(P<0.05);SSEA-1陽性細胞率和CD133陽性細胞率呈正相關關系(Cp=0.84, P<0.05);CD133陽性細胞率和Nestin陽性細胞率呈正相關關系(Cp=0.48,P<0.05);SSEA-1陽性細胞率和Nestin陽性細胞率呈正相關系(Cp=0.44,P<0.05)。 結論:①人腦膠質瘤組織中在不同病理級別的膠質瘤標本組中CD133、SSEA-1及Nestin表達程度不同; CD133、SSEA-1、Nestin的表達與膠質瘤病理分級呈正相關,提示三者的高表達與膠質瘤的惡性程度有關。②檢測腦膠質瘤組織中CD133、SSEA-1、Nestin的表達有助于膠質瘤患者的診斷及惡性程度判斷。③CD133、SSEA-1和Nestin作為膠質瘤干細胞標記物在膠質瘤的診斷和治療以及評估預后和未來個性化綜合治療中發(fā)揮重大的作用。

Although the current surgery, radiotherapy and chemotherapy have been greatly improved, but for the treatment and prognosis of glioma, it is far from the ideal effect. The proposal of glioma stem cells provides us with a new way of thinking. The purpose of this study was to study the expression of glioma stem cell markers CD133,SSEA-1 (stage specific embryonic antigen-1) and Nestin (nestin) in human glioma tissues and their correlation with pathological grade, to explore the clinical significance of the three markers in the diagnosis and malignant degree of human glioma, and to provide the theoretical basis of molecular biology for treatment. Methods: from September 2011 to December 2012, glioma was confirmed by pathology in the affiliated Hospital of Guangxi Medical University.
