【摘要】：背景:雖然原發(fā)性腦出血（ICH）在全部卒中類型中所占比例較缺血性卒中低，但卻與高的致死率和致殘率相關(guān)，易造成永久性的神經(jīng)功能障礙。前期研究對影響腦出血遠(yuǎn)期預(yù)后的因素進(jìn)行了較多報(bào)道，但對短期預(yù)后的報(bào)道較少。 目的:探討原發(fā)性腦出血患者短期預(yù)后的影響因素。 方法:回顧性分析我院235例發(fā)病后在60天內(nèi)出院的急性原發(fā)性腦出血患者的既往史、臨床、影像學(xué)、實(shí)驗(yàn)室數(shù)據(jù)、內(nèi)科或外科治療、并發(fā)癥等相關(guān)資料，通過MRS評分評價(jià)患者出院時(shí)的預(yù)后情況。采用多因素Logistic回歸方法分析與短期預(yù)后獨(dú)立相關(guān)的因素。 結(jié)果:共235名原發(fā)性腦出血患者，出院時(shí)預(yù)后良好組為128例（54.5%），預(yù)后不良組為107例（45.5%）。在多因素Logistic回歸分析中，年齡（OR=1.071，p0.001）、GCS評分（OR=0.755，p=0.024）、GCS≤8分（OR=0.101，，p=0.013）、入院時(shí)NIHSS評分（OR=1.169，p=0.001）、出血破入腦室（OR=3.356， p=0.003）、中線結(jié)構(gòu)移位（OR=2.894，p=0.014）是短期預(yù)后的影響因素（p0.05）。 結(jié)論:45.5%的原發(fā)性腦出血患者在發(fā)病60天內(nèi)出院時(shí)預(yù)后不良；年齡、GCS評分（GCS評分≤8分）、入院時(shí)NIHSS評分、出血破入腦室、中線結(jié)構(gòu)移位是影響原發(fā)性腦出血患者短期預(yù)后的獨(dú)立危險(xiǎn)因素。
[Abstract]:Background: although the proportion of primary cerebral hemorrhage (ICH) in all stroke types is lower than that in ischemic stroke, it is associated with high mortality and disability rate and is prone to permanent neurological dysfunction. Previous studies have reported many factors affecting the long-term prognosis of intracerebral hemorrhage, but few have been reported on short-term prognosis. Objective: to explore the influencing factors of short-term prognosis in patients with primary cerebral hemorrhage. Methods: the history, clinical, imaging, laboratory data, internal or surgical treatment and complications of 235 patients with acute primary cerebral hemorrhage who were discharged from our hospital within 60 days after onset were retrospectively analyzed. The prognosis of patients at discharge was evaluated by MRS score. Multivariate Logistic regression was used to analyze independent factors associated with short-term prognosis. Results: of 235 patients with primary cerebral hemorrhage, 128 (54.5%) had a good prognosis and 107 (45.5%) had a poor prognosis.
