【摘要】：目的考察雙側(cè)上肢等速肌力訓(xùn)練結(jié)合針刺對(duì)腦卒中肩關(guān)節(jié)半脫位患者肩關(guān)節(jié)肌群表面肌電的影響及療效。方法腦卒中肩關(guān)節(jié)半脫位患者120例按入院順序隨機(jī)分為對(duì)照組和觀察組各60例。對(duì)照組給予針刺治療,觀察組給予雙側(cè)上肢等速肌力訓(xùn)練結(jié)合針刺治療。結(jié)果與治療前比較,角速度為60°/s時(shí),兩組治療后三角肌和岡肌積分肌電值(i EMG)顯著降低(P0.05);與對(duì)照組比較,觀察組降低明顯(P0.05);角速度為90°/s和120°/s時(shí),兩組三角肌和岡肌i EMG明顯增高(P0.05);與對(duì)照組比較,觀察組增高明顯(P0.05)。治療后,兩組雙側(cè)肩峰至肱骨頭間距明顯減少(P0.05),兩側(cè)肩胛下角頂點(diǎn)間距明顯增大(P0.05);與對(duì)照組比較,觀察組更明顯(P0.05)。與對(duì)照組比較,觀察組治療顯效例數(shù)明顯增多(P0.05)。結(jié)論雙側(cè)上肢等速肌力訓(xùn)練結(jié)合針刺對(duì)腦卒中肩關(guān)節(jié)半脫位有確切療效,可能與影響肩關(guān)節(jié)肌群表面肌電有關(guān)。
[Abstract]:Objective to investigate the effect of isokinetic muscle strength training combined with acupuncture on the surface myography of shoulder muscles in patients with shoulder subluxation after stroke. Methods 120 patients with shoulder subluxation after stroke were randomly divided into control group (n = 60) and observation group (n = 60). The control group was treated with acupuncture, and the observation group was treated with bilateral isokinetic muscle strength training combined with acupuncture. Results compared with those before treatment, the (i EMG) of deltoid muscle and goniornus muscle in both groups decreased significantly after treatment when angular velocity was 60 擄/ s (P0.05), and that in observation group was significantly lower than that in control group (P0.05). When the angular velocity was 90 擄/ s and 120 擄/ s, the I EMG of deltoid and gonadal muscles increased significantly in both groups (P0.05), and that in the observation group was significantly higher than that in the control group (P0.05). After treatment, the distance between shoulder peak and humerus head in both groups decreased significantly (P0.05), and the distance between the two sides of the subscapular angle increased significantly (P0.05); compared with the control group, the observation group was more obvious (P0.05). Compared with the control group, the treatment in the observation group significantly increased the number of cases (P0.05). Conclusion bilateral upper limb isokinetic muscle strength training combined with acupuncture is effective in the treatment of shoulder subluxation after stroke, which may be related to the influence of surface myography on shoulder muscle group.
