【摘要】：目的分析磁共振三維動脈自旋標(biāo)記成像(3D-ASL)在急性缺血性腦梗死患者中的應(yīng)用價值。方法 22例缺血性腦梗死患者行常規(guī)MRI序列、DWI、MRA、3D-ASL檢查。采集原始數(shù)據(jù)上傳至ADW 4.2工作站,采用Func-tool軟件處理,獲取灌注參數(shù)的偽彩圖。觀察腦梗死患者的ASL-CBF、DWI圖像并分別測量異常灌注面積及梗死面積,分別用Excel表格記錄DWI及ASL所測數(shù)值,統(tǒng)計數(shù)值均以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,采用兩獨立樣本t檢驗比較兩者間的差異;觀察MRA梗死區(qū)周圍的腦血管有無狹窄及閉塞。結(jié)果 22例患者中19例ASL顯示不同程度灌注異常,19例灌注異�；颊叩漠惓９嘧⒉课痪cDWI梗死灶一致,ASL-CBF圖上灌注異常面積為(17.10±6.15)cm~2,DWI梗死面積為(13.54±4.97)cm~2,ASL-CBF圖上灌注異常面積大于DWI圖上梗死面積,且差異有統(tǒng)計學(xué)意義(P=0.029)。結(jié)論 3D-ASL在臨床應(yīng)用中能夠較可靠地反映缺血性腦梗死患者的低灌注狀態(tài),結(jié)合MRA及DWI更有利于指導(dǎo)臨床診療。
[Abstract]:Objective to analyze the value of magnetic resonance three-dimensional arterial spin-label imaging (3D-ASL) in patients with acute ischemic cerebral infarction. Methods 22 patients with ischemic cerebral infarction were examined by conventional MRI sequence. The original data was uploaded to ADW 4.2 workstation and processed by Func-tool software to obtain pseudo-color images of perfusion parameters. The ASL-CBF,DWI images of patients with cerebral infarction were observed and the abnormal perfusion area and infarct area were measured respectively. The values of DWI and ASL were recorded by Excel tables. The statistical values were expressed as mean 鹵standard deviation (x 鹵s). Two independent samples t test were used to compare the difference between the two groups and to observe the stenosis and occlusion of cerebral vessels around the infarct area of MRA. Results 19 cases of ASL showed abnormal perfusion in different degree. All the abnormal perfusion sites of 19 cases were consistent with the infarct focus of DWI. The area of abnormal perfusion on ASL-CBF map was (17.10 鹵6.15) cm~2,DWI. The area of abnormal perfusion on cm~2,ASL-CBF was (13.54 鹵4.97). The area of abnormal perfusion on ASL-CBF was (13.54 鹵4.97). Infarct area on DWI, The difference was statistically significant (P0. 029). Conclusion 3D-ASL can reliably reflect the hypoperfusion status of patients with ischemic cerebral infarction in clinical application, and the combination of MRA and DWI is more helpful for clinical diagnosis and treatment.
