【摘要】：目的分析缺血性腦卒中亞型與幽門螺桿菌感染相關性,為該病的臨床治療提供指導。方法回顧性分析2016年1-10月期間在本院住院的179例缺血性腦卒中患者(病例組)和同時期193例健康體檢者(對照組)幽門螺桿菌感染情況;采用CISS腦卒中分型方法對病例組進行缺血性腦卒中分型,分析缺血性卒中亞型與幽門螺桿菌感染的相關性。結果分析人口統(tǒng)計學特征及臨床資料,病例組和對照組患者高血壓病史、糖尿病史、吸煙史、家族卒中病史對比的OR值1,性別、生活規(guī)律、消化性潰瘍病史、高脂血癥、教育程度對比的OR值1。病例組幽門螺桿菌感染127例,感染率為70.95%;對照組幽門螺桿菌感染109例,感染率為56.48%。幽門螺桿菌感染可能是缺血性腦卒中患者發(fā)病的危險因素(OR值為1.88,95%CI值為1.22~2.89)。采用CISS分型法對缺血性腦卒中進行分型,主要類型為LAA、CS、PAD、OE和UE。其中3種主要亞型LAA型感染90例,感染率為70.87%,OR值為1.65;CS型感染25例,感染率為19.69%,OR值為0.91;PAD感染12例,感染率為9.45%,OR值為0.44。幽門螺桿菌感染可能是LAA型缺血性腦卒中發(fā)生的危險因素。結論高血壓病史、糖尿病史、吸煙史、家族卒中病史可能增加發(fā)生缺血性腦卒中的風險,幽門螺桿菌感染可能是LAA型缺血性腦卒中發(fā)生的危險因素。
[Abstract]:Objective to analyze the relationship between subtype of ischemic stroke and Helicobacter pylori infection in order to provide guidance for clinical treatment of ischemic stroke. Methods the infection of Helicobacter pylori in 179 patients with ischemic stroke (case group) and 193 healthy persons (control group) hospitalized in our hospital from January to October 2016 were analyzed retrospectively. The CISS method was used to type ischemic stroke and the correlation between the subtype of ischemic stroke and Helicobacter pylori infection was analyzed. Results the OR values of hypertension, diabetes mellitus, smoking history, family stroke history, sex, life pattern, peptic ulcer history, hyperlipidemia were analyzed. The OR value of educational level contrast is 1. 5%. There were 127 cases of Helicobacter pylori infection in the case group and 109 cases of Helicobacter pylori infection in the control group. Helicobacter pylori infection may be a risk factor for ischemic stroke patients (OR = 1.88 / 95, CI = 1.22 / 2.89). The main types of ischemic stroke were LAA,CS,PAD,OE and UE. by using CISS classification method. Among them, 90 cases were infected with three main subtypes of LAA, the infection rate was 70.87% and the OR value was 1.65%. The infection rate was 19.69% (OR = 0.91%) in 12 cases, and the infection rate was 9.45% (OR = 0.44). Helicobacter pylori infection may be a risk factor for LAA ischemic stroke. Conclusion the history of hypertension, diabetes, smoking and family stroke may increase the risk of ischemic stroke. Helicobacter pylori infection may be the risk factor of LAA ischemic stroke.
