磁敏感加權成像血管結構半定量評分法對顱內膠質瘤分級的診斷價值

【摘要】:目的探討磁敏感加權成像(susceptibility weighted imaging,SWI)血管結構半定量評分法對顱內膠質瘤分級的診斷價值。 方法回顧性分析36例(15例低級別膠質瘤和21例高級別膠質瘤)膠質瘤患者的影像資料,患者于術前行磁共振常規(guī)平掃、增強和SWI掃描,SWI原始圖像經(jīng)后處理后得到最小強度投影圖(minimum intensity projection,MinIP)。采用盲法,兩名高年資神經(jīng)影像診斷醫(yī)師分別觀察SWIminIP圖腫瘤內新生血管數(shù)目并進行評分,,分析兩組膠質瘤血管評分有無差異。 結果15例低級別膠質瘤,6例血管評分為0分,9例血管評分為1分;21例高級別膠質瘤,18例血管評分為2分,2例血管評分為3分,1例血管評分為1分。低級別膠質瘤組與高級別膠質瘤組血管評分有統(tǒng)計學意義(Z值為-5.327,P值0.05)。 結論磁敏感加權成像血管結構半定量評分法有助于顱內高、低級別膠質瘤的初步分級診斷。
[Abstract]:Objective to evaluate the diagnostic value of magnetic sensitivity weighted imaging (susceptibility weighted imagingsSWI) in the diagnosis of intracranial gliomas. Methods the imaging data of 36 cases of gliomas (15 cases of low grade gliomas and 21 cases of high grade gliomas) were retrospectively analyzed. After post-processing, the minimum intensity projection (minimum intensity projectionMinIP was obtained. The number of neovascularization in glioma was observed by two senior neuroimaging diagnostics with blind method and the difference between the two groups was analyzed. Results in 15 cases of low grade glioma, 6 cases had 0 score, 9 cases had 1 score, 21 cases had high grade glioma, 18 cases had blood vessel score, 2 cases had 3 points, 1 case had blood vessel score, 1 case had blood vessel score. There was significant difference in blood vessel score between low grade glioma group and high grade glioma group (Z value -5.327 P value 0.05). Conclusion the semi-quantitative scoring method of vascular structure in magnetic sensitivity weighted imaging is helpful for the initial grade diagnosis of high and low grade intracranial gliomas.
