【摘要】：目的:探討彌散加權(quán)成像(diffusion-weighted imaging,DWI)在預(yù)測(cè)成膠質(zhì)細(xì)胞瘤(glioblastoma,GBM)患者放化療預(yù)后中的價(jià)值。方法:對(duì)GBM肉眼全切患者進(jìn)行前瞻性研究。所有患者均接受標(biāo)準(zhǔn)放化療。術(shù)后放療前進(jìn)行DWI檢查,測(cè)量近瘤周區(qū)、中瘤周區(qū)和遠(yuǎn)瘤周區(qū)的表觀彌散系數(shù)(apparent diffusion coefficient,ADC)值,分別按ADC平均值和ADC最小值的中位值進(jìn)行分組,分析患者的總生存(overall survival,OS)和無進(jìn)展生存(progression-free survival,PFS)情況。結(jié)果:共83例患者入組,6個(gè)月和12個(gè)月的OS率分別為99%和82%,6個(gè)月和12個(gè)月的PFS率分別為84%和66%。近瘤周區(qū)ADC平均值0.979×10-3 mm2/s組的OS和PFS均長(zhǎng)于ADC平均值≤0.979×10-3 mm2/s(P OS=0.019,P PFS=0.005);該區(qū)ADC最小值0.894×10-3 mm2/s組的PFS長(zhǎng)于ADC最小值≤0.894×10-3 mm2/s組(P=0.018)。在中瘤周區(qū)和遠(yuǎn)瘤周區(qū),各組OS和PFS間的差異均無統(tǒng)計(jì)學(xué)意義(P值均0.05)。結(jié)論:DWI對(duì)預(yù)測(cè)GBM患者的放化療療效有一定價(jià)值。
[Abstract]:Objective: to investigate the value of diffusion-weighted imaging (DWI) in predicting the prognosis of glioblastoma GBM (glioblastomaGBM) patients. Methods: a prospective study was carried out on patients with GBM gross ophthalmectomy. All patients received standard radiotherapy and chemotherapy. DWI was performed before radiotherapy, and the apparent diffusion coefficients (ADCs) were measured in the peri-neoplastic area, the middle peri-tumour area and the paratumoral area. The ADCs were grouped according to the mean value of ADC and the median value of the minimum value of ADCs, respectively, according to the mean value of ADC and the median value of the minimum value of ADC. The total survival rate (OS) and progression-free survival (PFS) were analyzed. Results: the OS rates were 99% and 82% for 6 months and 12 months, and 84% and 66% for 6 months and 12 months, respectively.
