【摘要】：目的采用經(jīng)顱多普勒超聲(TCD)發(fā)泡試驗(yàn)研究前庭性偏頭痛(VM)患者心臟右向左分流(RLS)的發(fā)生率。方法收集2016年6月至2017年1月就診于陜西省人民醫(yī)院的VM患者的臨床資料。選取同期無心臟病史及反復(fù)發(fā)作頭痛史的健康志愿者為對(duì)照組。采集人口學(xué)資料及病史,行TCD發(fā)泡試驗(yàn)檢查。統(tǒng)計(jì)各組心臟RLS的發(fā)生率,以及RLS分流量與VM伴發(fā)癥狀的關(guān)系。結(jié)果納入VM患者139例(男23例,女116例);正常對(duì)照組30名。VM組和正常對(duì)照組的RLS發(fā)生率分別為25.9%、23.3%,兩組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。RLS陽性的VM患者出現(xiàn)心慌癥狀的比例高于RLS陰性的VM患者(P0.05)。視覺先兆在RLS陽性和陰性人群中的發(fā)生率差異無統(tǒng)計(jì)學(xué)意義(P0.05)。心臟RLS的分流量與VM的眩暈發(fā)病年齡、癥狀持續(xù)時(shí)間無顯著相關(guān)性(P0.05)。RLS陽性的VM患者中,小分流量組眩暈發(fā)作頻率為7.35次/年,而大分流量組為7.27次/年,兩組間差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 VM患者的RLS發(fā)生率與正常人群相比無明顯差異;小分流量RLS陽性的VM患者的癥狀發(fā)作更頻繁,RLS陽性VM患者出現(xiàn)心慌癥狀的比例也高于RLS陰性患者。
[Abstract]:Objective to study the incidence of right to left shunt (RLSs) in patients with vestibular migraine (VMM) by transcranial Doppler echocardiography (TCD). Methods from June 2016 to January 2017, the clinical data of VM patients in Shaanxi Provincial people's Hospital were collected. Data. Healthy volunteers with no history of heart disease and history of recurrent headache during the same period were selected as control group. Demographic data and medical history were collected. TCD foaming test was performed. The incidence of cardiac RLS and the relationship between RLS fractional flow and VM syndromes in each group were analyzed. Results 139 patients (23 males, 23 males) with VM were included in the study. The incidence of RLS in 30 cases of normal control group and 30 cases of normal control group was 25.9% and 23.3, respectively. There was no significant difference between the two groups in the incidence of panic symptoms in VM patients with positive P0.05. RLS-positive results were higher than those in patients with RLS negative VM patients (P 0.05). There was no significant difference in the incidence of RLS between positive and negative groups (P 0.05). The mean flow of cardiac RLS and the age of VM vertigo occurred. The frequency of vertigo attack was 7.35 times per year in the small flow group and 7.27 times / year in the large flow group. Conclusion there is no significant difference in the incidence of RLS between VM patients and normal controls. The frequency of symptomatic attack in patients with small flow RLS positive VM was also higher than that in RLS negative patients.
