急性缺血性腦卒中患者腦白質(zhì)病變與HbA1c水平的相關(guān)性分析

【摘要】:目的探討初發(fā)急性缺血性腦卒中患者腦白質(zhì)病變(cerebral white matter disease,WMD)的血管危險因素,及其與Hb A1c水平的相關(guān)性。方法納入初發(fā)急性缺血性腦卒中患者,收集患者入院當(dāng)時及入院后第2天的影像學(xué)資料和血生化檢查資料,根據(jù)患者WMD分級標(biāo)準(zhǔn)將患者分為4級。詳細記錄患者的人口統(tǒng)計學(xué)資料,血管危險因素和FLAIR序列顯示的WMD影像資料并進行比較和分析。結(jié)果共納入512例首發(fā)急性缺血性腦卒中患者,其中男性320(62.5%)例,460(89.8%)例患者存在WMD,其中無WMD患者52例,輕度WMD患者205例,中度WMD患者139例,重度WMD患者116例。單變量分析顯示,高齡、高血壓、腎小球濾過率降低,Hb A1c水平降低,男性與WMD呈正相關(guān)(P0.05)。進一步的多變量Logistic回歸發(fā)現(xiàn),高齡、高血壓和Hb A1c增高是WMD的獨立危險因素。結(jié)論急性缺血性腦卒中患者Hb A1c增高是WMD的獨立危險因素之一。
[Abstract]:Objective to investigate the vascular risk factors of white matter disease (cerebral white matter disease,WMD) in patients with primary acute ischemic stroke and its correlation with the level of Hb A1c. Methods the data of imaging and blood biochemical examination were collected from the patients with acute ischemic stroke at the time of admission and the second day after admission. The patients were divided into 4 grades according to the criteria of WMD classification. The demographic data, vascular risk factors and WMD data of FLAIR sequence were recorded and compared and analyzed. Results A total of 512 patients with first-episode acute ischemic stroke were enrolled, including 320 males (62.5%), 460 patients (89.8%) with WMD, 52 patients without WMD, 205 patients with mild WMD, and 139 patients with moderate WMD. There were 116 patients with severe WMD. Univariate analysis showed that old age, hypertension, glomerular filtration rate (GFR) and Hb A1c level decreased, and there was a positive correlation between male and WMD (P0.05). Further multivariate Logistic regression revealed that older age, hypertension and increased Hb A1c were independent risk factors for WMD. Conclusion the increase of Hb A1c in patients with acute ischemic stroke is one of the independent risk factors of WMD.
