【摘要】：目的提高臨床醫(yī)生對急性壞死性筋膜炎及陰囊壞疽的認(rèn)識。 方法結(jié)合肛周膿腫致大面積急性壞死性筋膜炎及陰囊壞疽一例的診治過程，復(fù)習(xí)相關(guān)文獻，對其病因、發(fā)病機制、臨床表現(xiàn)及診治特點進行分析。 結(jié)果急性壞死性筋膜炎及陰囊壞疽大多是由多因素致病，起病急，進展快，危害極大，處理相當(dāng)困難的軟組織感染性疾病。多數(shù)可經(jīng)多次及時手術(shù)切開清除壞死組織、創(chuàng)面沖洗換藥及抗休克、抗感染，加強營養(yǎng)支持等對癥治療后治愈。 結(jié)論急性壞死性筋膜炎及陰囊壞疽是罕見病，但近年來國內(nèi)外報道增多，，且發(fā)病早期與其他軟組織感染如丹毒、蜂窩織炎及氣性壞疽等難以鑒別，盡管現(xiàn)代醫(yī)學(xué)發(fā)展迅速，但誤診率仍較高，故早期診斷與及時適當(dāng)治療可明顯減少病死率與術(shù)后并發(fā)癥。
[Abstract]:Objective To improve the understanding of the clinical doctor about the acute necrotizing fasciitis and the gangrene of the scrotum. Methods The diagnosis and treatment of a large area of acute necrotizing fasciitis and a case of the scrotum gangrene were combined with perianal abscess. The relevant literature was reviewed, and the cause, the pathogenesis, the clinical manifestation and the characteristics of the diagnosis and treatment were analyzed. The results showed that the acute necrotizing fasciitis and the gangrene of the scrotum are mostly caused by multiple factors, the onset of the disease is acute, the progress is fast, the harm is great, and the soft tissue infection is very difficult to deal with. Most of the diseases can be treated in a number of times, such as the removal of necrotic tissue, the flushing of the wound surface, the anti-shock, the anti-infection, the strengthening of the nutritional support, and the like. Conclusion The acute necrotizing fasciitis and the gangrene of the scrotum are rare, but in recent years, the reports of acute necrotizing fasciitis and the gangrene of the scrotum are more and more difficult to distinguish between the early and other soft tissue infections such as erysipelas, cellulitis and gas gangrene. The rate of diagnosis is still high, so early diagnosis and proper treatment can significantly reduce the case fatality rate.
