【摘要】：目的： 觀察應(yīng)用中藥清瘟敗毒飲中西醫(yī)結(jié)合治療方案治療膿毒癥早期(治療3天)的臨床療效,以及對血乳酸、胱抑素C水平的影響,探索中藥清瘟敗毒飲在改善組織缺血缺氧、保護(hù)器官功能方面的作用,并探討清瘟敗毒飲治療膿毒癥的機(jī)制,為使用清瘟敗毒飲治療膿毒癥提供臨床證據(jù)。 方法： 選取符合膿毒癥診斷標(biāo)準(zhǔn)、中醫(yī)辨證為氣血兩燔證的患者53例,將53例患者隨機(jī)(隨機(jī)排列表法)分為兩組,治療組26例在西醫(yī)綜合治療方法基礎(chǔ)上加用“清瘟敗毒飲”,對照組27例僅給予西醫(yī)綜合方法治療,療程3天,觀察兩組患者的中醫(yī)癥狀積分、APACHE II評分、血乳酸、胱抑素C等指標(biāo)動態(tài)變化。 結(jié)果： 3天后治療組在改善中醫(yī)癥狀積分、APACHE II評分、血乳酸、血胱抑素C水平方面與對照組相當(dāng),差異沒有統(tǒng)計學(xué)意義。 結(jié)論： 應(yīng)用清瘟敗毒飲治療膿毒癥在3天內(nèi)未能有效提高膿毒癥臨床療效,治療早期在改善組織缺血缺氧、保護(hù)腎臟功能方面與對照組相比無差異：
[Abstract]:Objective: to observe the clinical effect of traditional Chinese medicine Qingwen Duduyin on the early stage (3 days of treatment) of sepsis and the effect on the level of blood lactic acid and cystatin C (Cystatin C). To explore the role of Qingwen Gudu Yin in improving tissue ischemia and hypoxia and protecting organ function, and to explore the mechanism of Qingwen Duduyin in the treatment of sepsis, so as to provide clinical evidence for the treatment of sepsis with Qingwen Duduyin. Methods: 53 cases of sepsis were randomly divided into two groups according to the diagnosis criteria of sepsis and syndrome differentiation of qi and blood, and 53 cases were randomly divided into two groups. Twenty-six cases in the treatment group were treated with "Qingwen Di du Yin" on the basis of the comprehensive treatment of western medicine, and 27 cases in the control group were treated only with the western medicine comprehensive method. The course of treatment was 3 days. The, APACHE II score and blood lactic acid of the two groups of patients with symptoms of traditional Chinese medicine were observed. The changes of cystine C and other indexes were dynamic. Results: after 3 days, the scores of, APACHE II, blood lactic acid and cystatin C in the treatment group were similar to those in the control group, and there was no significant difference between the treatment group and the control group. Conclusion: Qingwen Duduyin can not effectively improve the clinical efficacy of sepsis within 3 days. There is no difference between the early treatment group and the control group in improving tissue ischemia and hypoxia and protecting renal function.
