【摘要】：目的觀察急性缺血性卒中(AIS)患者行重組組織型纖溶酶原激活劑(rt-PA)靜脈溶栓治療后早期缺血加重的影響因素。方法選擇83例行rt-PA靜脈溶栓治療的AIS患者,根據24 h內是否出現缺血加重,分為加重組與非加重組。比較兩組臨床資料,采用多因素Logistic回歸分析早期缺血加重的影響因素。結果與非加重組相比較,加重組合并房顫及存在顱內大動脈嚴重狹窄的比例均較高(P均0.05)。合并房顫及顱內大動脈嚴重狹窄是靜脈溶栓治療后早期缺血加重有關(OR=20.77,95%CI為1.29~335.73;OR=14.14,95%CI為1.92~104.26)。結論合并房顫及存在顱內大動脈嚴重狹窄的AIS患者行靜脈溶栓后早期易出現缺血加重,應及早行血管內介入治療。
[Abstract]:Objective to investigate the influencing factors of early ischemic exacerbation after intravenous thrombolytic therapy with recombinant tissue plasminogen activator (rt-PA) in patients with acute ischemic stroke (AIS). Methods Eighty-three AIS patients who received intravenous thrombolytic therapy with rt-PA were divided into two groups according to whether ischemic exacerbation occurred within 24 hours. The clinical data of the two groups were compared. Multivariate Logistic regression analysis was used to analyze the influencing factors of early ischemic aggravation. Results compared with non-additive recombination, the proportion of aggravation and atrial fibrillation and severe intracranial artery stenosis were higher (P 0.05). Atrial fibrillation and severe stenosis of large intracranial artery were associated with early ischemic exacerbation after intravenous thrombolytic therapy (OR=20.77,95%CI = 1.29 335.73) and 14.1495 CI = 1.92 鹵104.26). Conclusion AIS patients with atrial fibrillation and severe intracranial artery stenosis are prone to ischemic exacerbation early after intravenous thrombolysis, and should be treated with intravascular interventional therapy as early as possible.
