老年急性缺血性腦卒中患者血清中尿酸、總膽紅素以及脂蛋白等相關(guān)因素的綜合性分析

【摘要】:目的探討老年急性缺血性腦卒中患者血清中尿酸、總膽紅素以及脂蛋白水平與其他因素的變化在急性缺血性腦卒中發(fā)生發(fā)展中的作用以及相關(guān)關(guān)系的綜合性分析。方法選取在2014年1月-2015年12月首次住院治療的老年急性缺血性腦卒中患者114例(腦卒中組)。急性期24h空腹抽血測(cè)定其急性期尿酸、總膽紅素、三酰甘油、血糖、高密度脂蛋白、低密度脂蛋白、載脂蛋白B(ApoB)、載脂蛋白A1(ApoA1)、總膽固醇等水平,與同期健康體檢者90例(對(duì)照組)進(jìn)行比較。結(jié)果急性缺血性腦卒中組體重指數(shù)(BMI)、尿酸、三酰甘油、血糖、低密度脂蛋白、ApoB/ApoA1、總膽固醇、收縮壓、舒張壓明顯高于對(duì)照組(P0.01);病例組總膽紅素和高密度脂蛋白低于對(duì)照組(P0.01);尿酸與BMI、三酰甘油、血糖、低密度脂蛋白、ApoB/ApoA1、舒張壓呈顯著正相關(guān)(P0.01),尿酸與總膽紅素和收縮壓呈負(fù)相關(guān)(P0.01);總膽紅素與高密度脂蛋白呈正相關(guān)(P0.01),與尿酸、三酰甘油、血糖、低密度脂蛋白、ApoB/ApoA1、總膽固醇、收縮壓以及舒張壓呈負(fù)相關(guān)(P0.01);低密度脂蛋白與BMI、尿酸、血糖、ApoB/ApoA1、總膽固醇呈正相關(guān)(P0.01),與年齡、三酰甘油、高密度脂蛋白呈負(fù)相關(guān)(P0.01);并且以上結(jié)果差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論急性腦卒中患者尿酸、總膽紅素以及脂蛋白水平的升高都是急性缺血性腦卒中的危險(xiǎn)因素,并且這3個(gè)獨(dú)立的危險(xiǎn)因素與其他相關(guān)因素協(xié)同作用,在老年急性缺血性腦卒中發(fā)生發(fā)展中起到了非常重要的作用。
[Abstract]:Objective to investigate the role of serum uric acid, total bilirubin, lipoprotein and other factors in the occurrence and development of acute ischemic stroke in elderly patients. Methods 114 elderly patients with acute ischemic stroke (stroke group) who were hospitalized for the first time from January 2014 to December 2015 were selected. The levels of uric acid, total bilirubin, triacylglycerol, blood glucose, high density lipoprotein, low density lipoprotein, apolipoprotein B (ApoB), apolipoprotein A1 (ApoA1), total cholesterol and so on were measured in 24 hours of acute phase. The results were compared with 90 healthy persons (control group) in the same period. Results (BMI), uric acid, triglyceride, blood glucose, low density lipoprotein, total cholesterol of ApoB/ApoA1, systolic blood pressure and diastolic blood pressure in acute ischemic stroke group were significantly higher than those in control group (P0.01). The total bilirubin and high density lipoprotein in the case group were lower than those in the control group (P0.01). Uric acid was positively correlated with BMI, triglyceride, blood glucose, low density lipoprotein, ApoB/ApoA1, diastolic blood pressure (P0.01), uric acid was negatively correlated with total bilirubin and systolic blood pressure (P0.01). Total bilirubin was positively correlated with high density lipoprotein (P0.01), negatively correlated with uric acid, triglyceride, blood glucose, low density lipoprotein, total cholesterol of ApoB/ApoA1, systolic blood pressure and diastolic blood pressure (P0.01). Low density lipoprotein was positively correlated with BMI, uric acid, blood glucose, total cholesterol of ApoB/ApoA1, (P0.01), and negatively correlated with age, triglyceride, and high density lipoprotein (P0.01). And the above results were statistically significant (P0.05). Conclusion the elevated levels of uric acid, total bilirubin and lipoprotein in patients with acute stroke are risk factors for acute ischemic stroke, and these three independent risk factors have synergistic effects with other related factors. It plays a very important role in the occurrence and development of acute ischemic stroke in the elderly.
