【摘要】：目的研究左西孟旦對老年急性心肌梗死合并心衰患者的臨床治療效果及對患者N端前腦鈉肽(NT-pro BNP)和炎癥反應(yīng)水平的影響。方法急性心肌梗死合并心衰患者80例,隨機分為對照組和觀察組,每組40例。對照組患者給予常規(guī)支持治療以及護理,觀察組患者在此基礎(chǔ)上加用左西孟旦,用藥24 h后對兩組患者的臨床療效和生化指標(biāo)進行評價。結(jié)果用藥后,兩組患者NT-pro BNP和超敏C反應(yīng)蛋白(hs-CRP)、白介素(IL)-6、腫瘤壞死因子(TNF)-α水平均明顯下降,與各組用藥前相比(P0.05);用藥后,觀察組患者NT-pro BNP和hs-CRP、IL-6、TNF-α水平均明顯低于對照組(P0.05);對照組總有效31例,總有效率為77.5%(31/40),觀察組總有效38例,總有效率為95.0%(38/40),兩組相比差異顯著(P0.05)。結(jié)論左西孟旦不僅可以顯著降低老年急性心肌梗死合并心衰患者的NT-pro BNP和hs-CRP、IL-6、TNF-α水平,改善臨床癥狀和體征,而且用藥過程中具有良好的安全性。
[Abstract]:Objective to study the clinical effect of levosimendan on elderly patients with acute myocardial infarction (AMI) complicated with heart failure (HF), and to study the effects of levosimendan on N-terminal forebrain natriuretic peptide (NT-pro BNP) and inflammatory response level in elderly patients with acute myocardial infarction. Methods 80 patients with acute myocardial infarction complicated with heart failure were randomly divided into control group and observation group with 40 cases in each group. The patients in the control group were given routine support therapy and nursing care, and the patients in the observation group were treated with levosimendan for 24 hours. The clinical efficacy and biochemical indexes of the two groups were evaluated. Results the levels of NT-pro BNP and hs-CRP, interleukin (IL) 6 and tumor necrosis factor (TNF)-偽 in the two groups were significantly decreased compared with those before treatment (P0.05). The levels of NT-pro BNP and hs-CRP,IL-6,TNF- 偽 in the observation group were significantly lower than those in the control group (P0.05), the total effective rate was 77.5% (31 / 40) in the control group, and the total effective rate was 95.0% (38 / 40) in the observation group (P0.05). Conclusion Levomycin can not only significantly reduce the levels of NT-pro BNP and hs-CRP,IL-6,TNF- 偽 in elderly patients with acute myocardial infarction and heart failure and improve clinical symptoms and signs, but also have good safety in the course of medication.
