【摘要】：目的探索卒中患者急性期單側(cè)空間忽略癥的發(fā)病率及其與病變部位、性質(zhì)的關(guān)系。方法所有入組患者接受蘋果刪除試驗篩查忽略癥,區(qū)分忽略癥的兩種形式,并比較以下3組人群中忽略癥的發(fā)病率:(1)出血性卒中患者與缺血性卒中患者;(2)左側(cè)大腦半球病變患者與右側(cè)大腦半球病變患者;(3)大腦半球病變患者與小腦、腦干病變患者。結(jié)果本研究入組169例卒中后患者,篩查出卒中后單側(cè)空間忽略癥患者34例(20.1%),包括19例以自我為中心的忽略癥,22例非自我為中心的忽略癥,相關(guān)分析提示兩種形式的忽略癥之間無明顯相關(guān)。忽略癥的發(fā)病率在缺血性卒中組和出血性卒中組、左側(cè)大腦半球和右側(cè)大腦半球病變組的比較差異均無統(tǒng)計學(xué)意義。而大腦半球病變患者忽略癥發(fā)病率則高于腦干、小腦病變患者。結(jié)論卒中后患者忽略癥發(fā)病率不低,兩種形式的忽略癥沒有明顯相關(guān)。忽略癥常見于大腦半球病變患者,但左右側(cè)半球病變沒有差別,而病變性質(zhì)對忽略癥發(fā)病沒有影響。
[Abstract]:Objective to explore the incidence of unilateral spatial neglect in acute stroke patients and its relationship with the location and nature of lesion. Methods all patients were screened by apple deletion test to distinguish the two forms of neglect. The incidence of neglect was compared in the following three groups:% 1) hemorrhagic stroke and ischemic stroke 2) left hemisphere lesion and right hemisphere lesion. Results among 169 patients with stroke, 34 patients with unilateral spatial neglect after stroke were screened out, including 19 patients with self-centered neglect and 22 patients with non-self-centered neglect. Correlation analysis showed that there was no significant correlation between the two forms of neglect. The incidence of neglect was found in ischemic stroke and hemorrhagic stroke. There was no significant difference between the left cerebral hemisphere and the right cerebral hemisphere, while the incidence of neglect was higher in the patients with cerebral hemisphere disease than in the brainstem and cerebellar lesion. Conclusion the incidence of neglect is not low in the patients with stroke. There is no significant correlation between the two forms of neglect, which are common in patients with cerebral hemispheric lesions, but there is no difference between the left and right hemispheric lesions, and the nature of the lesions has no effect on the pathogenesis of the disease.
