【摘要】：目的:分析并預(yù)測(cè)血尿素氮對(duì)呼吸危重癥患者死亡風(fēng)險(xiǎn)的預(yù)測(cè)價(jià)值,為臨床對(duì)疾病的預(yù)后及轉(zhuǎn)歸的預(yù)測(cè)提供幫助。方法:選擇入住呼吸重癥監(jiān)護(hù)病房(RICU)的患者共373名,根據(jù)患者的預(yù)后分為死亡組及存活組,對(duì)兩組患者進(jìn)行住院時(shí)間、APACHEⅡ評(píng)分及血尿素氮等檢查,分別采用T檢驗(yàn)、Logistic回歸分析及ROC曲線下面積等方法,分析影響RICU患者預(yù)后的相關(guān)因素。結(jié)果:患者存活率為71.05%,死亡率為28.95%。死亡組和存活組APACHEⅡ評(píng)分,入院及出院時(shí)白蛋白、血尿素氮,出院時(shí)肌酐比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。RICU住院時(shí)間、APACHEⅡ評(píng)分及血尿素氮是影響RICU患者預(yù)后的獨(dú)立因素,其中住院時(shí)間的回歸系數(shù)為0.013,優(yōu)勢(shì)比(OR)為1.013,(P0.05);APACHEⅡ評(píng)分的回歸系數(shù)為-0.117,優(yōu)勢(shì)比(OR)為0.890(P0.05);出院尿素回歸系數(shù)為-0.063,優(yōu)勢(shì)比(OR)為0.939(P0.05),以血尿素氮為10.65 mmol·L~(-1)截點(diǎn)分為低危組與高危組,對(duì)預(yù)后的預(yù)測(cè)最佳。結(jié)論:呼吸危重癥患者血尿素氮增加與預(yù)后不良相關(guān),RICU住院時(shí)間、APACHEⅡ評(píng)分及血尿素氮是影響RICU患者預(yù)后的獨(dú)立因素,RICU臨床醫(yī)生可根據(jù)BUN值及變化幅度對(duì)預(yù)后進(jìn)一步評(píng)估。
