【摘要】：目的探討關(guān)于斑點(diǎn)熱立克次體的臨床診斷及流行病學(xué)調(diào)查而進(jìn)行的診斷方法學(xué)研究。方法構(gòu)建R.Japonica Anhui 120 strain OmpB原核表達(dá)質(zhì)粒,表達(dá)、鑒定并純化重組蛋白,用其作為抗原建立間接ELISA檢測(cè)方法,檢測(cè)120份臨床樣本中日本斑點(diǎn)熱立克次體特異性IgG抗體,并評(píng)價(jià)方法的敏感性和特異性。結(jié)果獲得了高表達(dá)、高純度的OmpB重組蛋白,建立的間接ELISA方法體系與美國(guó)食品和藥物管理局認(rèn)可的診斷試劑盒相比有良好的特異性和敏感性,兩者的特異性為100%,敏感性為96.7%,符合率為99.2%,Kappa值為0.98。結(jié)論 OmpB重組蛋白具有很好的免疫反應(yīng)性,能特異地檢出斑點(diǎn)熱立克次體IgG抗體,間接ELISA方法可作為臨床診斷的技術(shù)儲(chǔ)備及流行病調(diào)查和科學(xué)研究。
[Abstract]:Objective to investigate the clinical diagnosis and epidemiological investigation of Rickettsia speckle fever. Methods the prokaryotic expression plasmid R.Japonica Anhui 120 strain OmpB was constructed, the recombinant protein was identified and purified, and the indirect ELISA assay was established to detect the specific IgG antibody of Rickettsia macularis in 120 clinical samples. The sensitivity and specificity of the method were evaluated. Results High expression and high purity of OmpB recombinant protein were obtained. The indirect ELISA method system had good specificity and sensitivity compared with the diagnostic kit approved by the Food and Drug Administration of the United States, and the specificity of the two methods was 100%. The sensitivity was 96.7%, the coincidence rate was 99.2%, and the Kappa value was 0.98. Conclusion OmpB recombinant protein has good immunoreactivity and can detect IgG antibody against Rickettsia speckle fever in different places. Indirect ELISA method can be used as a technical reserve for clinical diagnosis, epidemiological investigation and scientific research.
