【摘要】：目的觀察恩替卡韋對(duì)慢性乙型病毒性肝炎(CHB)患者血清乙肝病毒DNA(HBV-DNA)、乙肝表面抗原(HBs Ag)、乙肝e抗原(HBe Ag)定量的影響。方法選取CHB患者160例,均給予恩替卡韋0.5 mg/d空腹口服,持續(xù)治療96周。觀察所有患者治療前、治療12、24、48及96周后HBV DNA、HBs Ag、HBe Ag定量以及谷氨酸氨基轉(zhuǎn)移酶(ALT)水平,計(jì)算HBV DNA、HBe Ag轉(zhuǎn)陰率及ALT復(fù)常率,分析HBV DNA變化與HBs Ag、HBe Ag、ALT的相關(guān)性。結(jié)果隨治療時(shí)間延長,患者HBV DNA、HBe Ag定量及ALT水平逐漸降低(F=11.562、28.034、22.417,P0.01);HBe Ag轉(zhuǎn)陰率、HBV DNA轉(zhuǎn)陰率以及ALT復(fù)常率逐漸增加;治療前、治療12及24周時(shí),HBV DNA與HBs Ag滴度負(fù)相關(guān)(r=-0.383、-0.293、-0.224,P0.05或P0.01)、與HBe Ag滴度正相關(guān)(r=0.441、0.392、0.326,P0.05或P0.01)。結(jié)論恩替卡韋治療CHB療效確切,可有效促進(jìn)HBV DNA、HBe Ag轉(zhuǎn)陰率及ALT復(fù)常。治療早期,HBV DNA與HBs Ag負(fù)相關(guān)、與HBe Ag呈正相關(guān),治療后期無顯著相關(guān)性,治療過程中,應(yīng)聯(lián)合多項(xiàng)指標(biāo)綜合評(píng)價(jià)療效。
