【摘要】：目的通過遺傳關(guān)聯(lián)研究觀察慢性HBV感染免疫相關(guān)重要細(xì)胞因子IL-12、IL-1B、IL-17的基因多態(tài)性與恩替卡韋抗病毒治療HBe Ag血清學(xué)轉(zhuǎn)換之間的關(guān)聯(lián)。方法對109例恩替卡韋抗病毒治療的HBe Ag陽性慢性乙型肝炎患者每3個(gè)月進(jìn)行肝功能、乙肝標(biāo)志物、HBV DNA檢測,進(jìn)行HBe Ag血清學(xué)轉(zhuǎn)換評估,治療24個(gè)月時(shí)的應(yīng)答情況作為判定應(yīng)答與否的標(biāo)準(zhǔn)。發(fā)生HBe Ag血清學(xué)轉(zhuǎn)換者為應(yīng)答組;未發(fā)生HBe Ag血清學(xué)轉(zhuǎn)換者為無應(yīng)答組;分析IL-12A rs568408、IL-1B rs1143623、IL-17A rs8193036基因型與HBe Ag血清學(xué)轉(zhuǎn)換之間的關(guān)聯(lián)。結(jié)果 29例獲得HBe Ag血清學(xué)轉(zhuǎn)換(應(yīng)答組),80例未獲得HBe Ag血清學(xué)轉(zhuǎn)換(無應(yīng)答組),總體應(yīng)答率26.6%。IL-12A rs568408與HBe Ag血清學(xué)轉(zhuǎn)換顯著相關(guān),GA基因型發(fā)生HBe Ag血清轉(zhuǎn)換的易感性顯著高于GG基因型[OR=3.72(1.34~10.32),P=0.012]。未發(fā)現(xiàn)IL-1B rs1143623[OR=1.99(0.72~5.44),P=0.17]和IL-17A rs8193036[OR=1.54(0.32~7.33),P=0.60]與HBe Ag血清學(xué)轉(zhuǎn)換存在關(guān)聯(lián)。結(jié)論 IL-12A rs568408與慢性乙型肝炎恩替卡韋治療HBe Ag血清學(xué)轉(zhuǎn)換存在關(guān)聯(lián)。
[Abstract]:Objective to investigate the association between gene polymorphisms of immune-related cytokines IL-12,IL-1B,IL-17 and entecavir antiviral therapy for HBe Ag serological transformation in chronic HBV infection. Methods 109 patients with HBe Ag positive chronic hepatitis B treated with entecavir antiviral therapy were examined for liver function, hepatitis B marker, HBV DNA and HBe Ag serological conversion every 3 months. Responses at 24 months of treatment were used as criteria for determining responses. The patients with HBe Ag seroconversion were in response group and those without HBe Ag serological conversion were non-responders. The association between IL-12A rs568408,IL-1B rs1143623,IL-17A rs8193036 genotype and HBe Ag serological transformation was analyzed. Results HBe Ag serological conversion was obtained in 29 cases and HBe Ag serological conversion in 80 cases. The total response rate (26.6%.IL-12A rs568408) was significantly correlated with HBe Ag serological conversion. The susceptibility to HBe Ag seroconversion in GA genotype was significantly higher than that in GG genotype [OR=3.72 (1.3410. 32), P0. 012]. There was no association between IL-1B rs1143623 [OR=1.99 (0.72 ~ 5.44), P _ (0.17)] and IL-17A rs8193036 [OR=1.54 (0.32 ~ 7.33), P _ (0. 60)] and serological transformation of HBe Ag. Conclusion IL-12A rs568408 is associated with HBe Ag serological conversion in patients with chronic hepatitis B treated with entecavir.
