【摘要】：目的：分析慢性丙型肝炎（chronic hepatitis C，CHC）患者血清可溶性血管黏附蛋白-1（solublevascular adhesion protein，sVAP-1）水平與HCV RNA、ALT、AST、γ-GT、TBil、Alb及透明質(zhì)酸（hyaluronic acid，HA）水平的關(guān)系，探討sVAP-1作為致炎因子在CHC發(fā)病中的作用。 方法： 納入CHC患者90例作為病例組（CHC組），根據(jù)血清HCV RNA水平將患者分為2組：HCV-RNA水平陰性患者30例（CHC1組），HCV RNA水平陽性患者60例（CHC2組）；納入健康體檢者30例作為健康對照組（NC組）。 觀察對象均空腹取肘靜脈血，離心取上清，-70℃冰箱凍存，ELISA法檢測血清sVAP-1、HA水平；全自動(dòng)生化儀檢測肝功能；熒光定量PCR法檢測血清HCV RNA水平。正態(tài)分布數(shù)據(jù)以均數(shù)±標(biāo)準(zhǔn)差表示，組間比較采用t檢驗(yàn)或單因素方差分析；偏態(tài)分布數(shù)據(jù)以中位數(shù)表示，組間比較用Wilcoxon秩和檢驗(yàn)；相關(guān)性分析采用spearman檢驗(yàn)。 結(jié)果： 1. CHC組、NC組血清sVAP-1水平分別為：（148.54±48.60）和（72.23±35.82）ng/ml,組間比較差異有統(tǒng)計(jì)學(xué)意義（t=9.187，P=0.041）； 2. CHC組、NC組血清HA水平分別為：（140.09±47.96）和（75.07±24.09）ng/ml，組間比較差異具有統(tǒng)計(jì)學(xué)意義（t=4.047，，P=0.048）； 3. CHC患者中正常ALT組、ALT輕度升高組、ALT明顯升高組血清sVAP-1水平分別為：（96.69±29.48）、（128.88±21.81）和（187.79±30.04）ng/ml，組間比較有顯著性差異（F=84.30，P0.001）；HA水平分別為：（89.36±27.66）、（110.91±27.63）和（149.19±30.16）ng/ml，組間比較有顯著性差異（F=33.01，P 0.001）； 4. CHC2組血清sVAP-1水平較CHC1組為高，差異有統(tǒng)計(jì)學(xué)意義（P0.001）；高、中、低病毒水平組之間血清sVAP-1比較差異無統(tǒng)計(jì)學(xué)意義（P0.05）； 5. CHC患者血清sVAP-1水平與HA、ALT、AST、γ-GT、TBil均呈正相關(guān)(rs=0.804、0.661、0.715、0.372、0.434，P0.01），sVAP-1與Alb水平呈負(fù)相關(guān)（rs=-0.484，P 0.01），sVAP-1與HCV-RNA水平無相關(guān)（P0.05）。 結(jié)論： 1、sVAP-1可能與慢性丙型肝炎患者肝細(xì)胞炎性損傷及肝纖維化的發(fā)生密切相關(guān)。 2、慢性丙型肝炎患者血清sVAP-1水平與HCV復(fù)制有關(guān)。
[Abstract]:Objective: to analyze the relationship between serum soluble vascular adhesion protein (sVAP-1) and the levels of HCV RNA-ALTAST, 緯 -GTnTnTBil-Alb and hyaluronic acidha in patients with chronic hepatitis C (hepatitis), and to explore the role of sVAP-1 as an inflammatory factor in the pathogenesis of CHC. Methods: According to the serum HCV RNA level, 90 patients with CHC were divided into two groups: 30 patients with negative serum HCV-RNA level and 60 patients with positive RNA level in CHC1 group, and 30 healthy controls as NC group.
