【摘要】：目的評(píng)價(jià)中國(未包括香港、澳門特別行政區(qū)和臺(tái)灣地區(qū),下同)2014年脊髓灰質(zhì)炎(脊灰)實(shí)驗(yàn)室網(wǎng)絡(luò)(Polio Laboratory Network,PLN)運(yùn)轉(zhuǎn)情況,為中國維持無脊灰狀態(tài)提供病毒學(xué)依據(jù)。方法對(duì)中國急性弛緩性麻痹(Acute flaccid paralysis,AFP)病例監(jiān)測(cè)信息報(bào)告管理系統(tǒng)中31個(gè)省份報(bào)告的AFP病例和國家脊灰實(shí)驗(yàn)室(National Polio Laboratory,NPL)的監(jiān)測(cè)數(shù)據(jù)進(jìn)行分析,對(duì)PLN的各項(xiàng)運(yùn)轉(zhuǎn)指標(biāo)進(jìn)行評(píng)價(jià)。結(jié)果 2014年中國PLN共報(bào)告5 751例AFP病例,收到5 688例AFP病例的11 363份糞便標(biāo)本;14天內(nèi)雙份糞便標(biāo)本的采集率為92.16%,合格糞便標(biāo)本采集率為91.65%,病毒分離結(jié)果 18天內(nèi)反饋率為97.62%。從5 688例糞便標(biāo)本中132例(2.32%)分離到脊灰病毒(Poliovirus,PV);590例(10.37%)分離到非脊灰腸道病毒(NPEV)。2014年NPL收到省級(jí)脊灰實(shí)驗(yàn)室送檢的分離自AFP病例及其接觸者、非AFP病例、健康人的292株L20 B細(xì)胞陽性分離物,262株為PV,21株為NPEV,9株為非腸道病毒(Non-enterovirus,NEV)。262株P(guān)V中,7株為疫苗衍生脊灰病毒(Vaccine-derived poliovirus,VDPV),來自內(nèi)蒙古、山西、河北和湖南;未發(fā)現(xiàn)脊灰野病毒(Wild poliovirus,WPV)。從脊灰病毒外環(huán)境監(jiān)測(cè)采集的污水標(biāo)本中分離到4株VDPV。2014年NPL和31個(gè)省級(jí)CDC脊灰實(shí)驗(yàn)室都通過了世界衛(wèi)生組織(WHO)糞便標(biāo)本腸道病毒的新檢測(cè)流程的能力驗(yàn)證標(biāo)本考核。NPL和8個(gè)省級(jí)CDC脊灰實(shí)驗(yàn)室接受并通過了WHO現(xiàn)場(chǎng)評(píng)估認(rèn)證,其他省級(jí)實(shí)驗(yàn)室接受了信函認(rèn)證。結(jié)論中國PLN 2014年運(yùn)轉(zhuǎn)正常,未發(fā)現(xiàn)WPV,為維持中國無脊灰狀態(tài)、早期發(fā)現(xiàn)VDPV或VDPV病例、早期采取應(yīng)對(duì)措施阻斷其傳播提供了重要的病毒學(xué)依據(jù)。
[Abstract]:Objective to evaluate the operation of Polio Laboratory Network (PLN) in China (excluding Hong Kong, Macao Special Administrative region and Taiwan) in 2014, and to provide virological evidence for maintaining poliomyelitis in China.Methods the data of AFP cases reported in 31 provinces and National Polio Laboratory-NPLs of National Polio Laboratory in China acute flaccid paralysis (AFP) case monitoring information reporting system were analyzed, and the operation indexes of PLN were evaluated.Results in 2014, 5 751 cases of AFP were reported by PLN in China, 11 363 stool specimens of 5 688 cases of AFP were received in 2014. The collection rate of double stool specimens was 92.16 in 14 days, the collection rate of qualified stool specimens was 91.65 and the feedback rate of virus isolation results within 18 days was 97.62.Poliovirus PVV (590 cases) were isolated from 5 688 fecal specimens. (NPL received AFP cases from provincial polio laboratories and their contacts, non- cases in 2014.292 strains of L20B cell positive isolates of healthy people, including 21 strains of PVN, 21 strains of NPEV, 9 strains of non-enterovirus.262 strains of PV, 7 strains of vaccine-derived poliovirusus virus (VDPVN), from Inner Mongolia, Shanxi, Hebei and Hunan, and no wild poliovirus wild poliovirusus PVV.Four strains of VDPV.2014 annual NPL and 31 provincial CDC polio laboratories were isolated from sewage samples collected from the external environment monitoring of poliovirus. All of them passed the new detection procedure of the World Health Organization (WHO) fecal specimens.Nuclear. NPL and 8 provincial CDC polio laboratories accepted and passed WHO field assessment certification.Other provincial laboratories were certified by correspondence.Conclusion PLN in China has not been found in 2014, which provides an important virological basis for the maintenance of poliomyelitis in China, the early detection of VDPV or VDPV cases and the early adoption of countermeasures to block their transmission.
