【摘要】：目的了解南京地區(qū)無償獻(xiàn)血者HBV感染人群分布情況,分析HBV在不同性別、年齡、城鄉(xiāng)差異等方面的感染情況。方法選取南京紅十字血液中心2015年01月-2016年01月期間金標(biāo)乙肝試紙快檢合格后的無償獻(xiàn)血者的血液標(biāo)本66 742份,分別行ELISA和NAT篩查實(shí)驗(yàn),對無償獻(xiàn)血者進(jìn)行年齡、性別、教育程度、民族、婚姻狀況、區(qū)域、獻(xiàn)血情況與HBV篩查陽性結(jié)果分析。結(jié)果本研究66 742例無償獻(xiàn)血者中,HBV篩查總陽性225例(0.34%),ELISA陽性180例(0.27%),NAT陽性45例(0.07%);無償獻(xiàn)血者HBV陽性率與年齡段、性別,教育程度和婚姻狀況無關(guān)(P0.05);而不同地區(qū)無償獻(xiàn)血者在HBV陽性檢出率,城市明顯高于農(nóng)村(χ~2=4.496,P0.05);漢族與其他民族相比,無償獻(xiàn)血者HBV陽性率較高,且差別有統(tǒng)計(jì)學(xué)意義(χ~2=4.523,P0.05)。結(jié)論南京地區(qū)無償獻(xiàn)血者HBV陽性率仍有一定比例,使用NAT方法可以大大縮短"窗口期",顯著降低因ELISA漏檢而導(dǎo)致的輸血后乙肝病毒感染發(fā)生率,需大力普及ELISA+NAT的應(yīng)用,進(jìn)一步降低輸血過程中乙肝病毒的傳播風(fēng)險(xiǎn)。
[Abstract]:Objective to investigate the distribution of HBV infection among unpaid blood donors in Nanjing, and to analyze HBV in different sex and age. Methods from January 2015 to January 2016, 66 742 blood samples from eligible blood donors were collected from Nanjing Red Cross Blood Center. ELISA and NAT screening tests were performed respectively. Age, sex, education, nationality, marital status, region of unpaid blood donors, Results in this study, 225 cases were positive for HBV screening in 66,742 volunteer donors. Results the positive rate of HBV, age and sex were 0.27%, 0.27%, 0.27%, 0.27%, 0.27%, 0.27%, 0.27%, 0.27% and 0.07%, respectively, among the 66,742 unpaid blood donors in this study, and the positive rate, age and sex of free blood donors were also found in this study. There was no correlation between education level and marital status, but the positive rate of HBV was significantly higher in urban than in rural areas among unpaid blood donors in different areas (蠂 2 449 6 P 0.05). The positive rate of HBV in the Han nationality was higher than that in other ethnic groups. The difference was statistically significant (蠂 ~ (2 +) 4.523) P0.050.Conclusion there is still a certain proportion of HBV positive rate in unpaid blood donors in Nanjing area. The "window period" can be greatly shortened by using NAT method, and the incidence of hepatitis B virus infection after transfusion caused by missed ELISA examination can be significantly reduced. It is necessary to popularize the application of ELISA NAT to further reduce the risk of hepatitis B virus transmission during blood transfusion.
