系統(tǒng)性紅斑狼瘡合并真菌感染患者NK細(xì)胞功能的變化

【摘要】:研究背景和研究目的:系統(tǒng)性紅斑狼瘡是一種以高滴度的自身抗體、循環(huán)免疫復(fù)合物升高以及補(bǔ)體下降為主要特點(diǎn),多種臨床表現(xiàn)為主要特征,多因素參與的自身免疫性疾病,其發(fā)病機(jī)制未明。而NK細(xì)胞是固有免疫的重要組成部分,是抵抗外來(lái)病原體侵犯的第一道防線。本實(shí)驗(yàn)的目的主要是分析NK細(xì)胞在SLE患者中免疫防御功能的改變。 研究方法:收集2013年8月至2014年4月在中南大學(xué)湘雅醫(yī)院確診的SLE. SLE合并肺部真菌感染、肺部真菌感染患者及健康體檢者的外周血。將實(shí)驗(yàn)組分為四組,即SLE組(n=20)、SLE合并肺部真菌感染組(n=14)、肺部真菌感染組(n=14)和健康對(duì)照組(n=20),用流式細(xì)胞術(shù)分別檢測(cè)各組患者和健康人外周血中CD3-CD56+NK細(xì)胞數(shù)、NK表面受體NKp44和TLR2、胞內(nèi)顆粒酶B的表達(dá)。 實(shí)驗(yàn)結(jié)果:在SLE與SLE合并肺部真菌感染患者外周血中,NK細(xì)胞數(shù)目較其他兩組明顯下降(P0.05),而兩組間比較無(wú)顯著差異(P0.05),肺部真菌感染組較正常組升高,差異無(wú)顯著性(P0.05);顆粒酶B在SLE組外周血中的表達(dá)較健康對(duì)照組及肺部真菌感染組明顯減少(P0.05),而在SLE合并肺部真菌感染組中,顆粒酶B表達(dá)較健康對(duì)照組明顯增高(P0.05),且較SLE組和肺部真菌感染組也明顯增高(P0.05),肺部真菌感染組較健康對(duì)照組無(wú)明顯差異;NKp44在SLE合并肺部真菌感染患者中NK細(xì)胞的表達(dá)較其他三組明顯升高(P0.05); TLR2在SLE組及SLE合并肺部真菌感染組中的表達(dá)較健康對(duì)照組及肺部真菌感染組明顯升高(P0.05),SLE合并肺部真菌感染組較SLE組表達(dá)減少,而肺部真菌感染組較健康對(duì)照組明顯增加(P0.05)。 結(jié)論:在系統(tǒng)性紅斑狼瘡患者體內(nèi),NK細(xì)胞及顆粒酶B的比例較正常組明顯減少,其免疫防御功能降低;TLR2的表達(dá)在SLE患者合并肺部真菌感染組中較SLE組降低,而在肺部真菌感染組中的表達(dá)較正常組增高;系統(tǒng)性紅斑狼瘡患者合并真菌感染時(shí),NKp44表達(dá)較健康組及SLE組上調(diào),并伴有TLR2和顆粒酶B的明顯升高,NK細(xì)胞功能被激活。
[Abstract]:Background and objective: systemic lupus erythematosus (SLE) is characterized by high titer autoantibodies, elevated circulating immune complex and decreasing complement. The pathogenesis of multifactorial autoimmune diseases is not clear, and NK cells are an important part of innate immunity. It is the first line of defense against the invasion of foreign pathogens. The purpose of this study was to analyze the changes of immune defense function of NK cells in patients with SLE. Methods: collected from August 2013 to April 2014 in Xiangya Hospital of Central South University diagnosed SLE. SLE with pulmonary fungal infection. Peripheral blood of patients with pulmonary fungal infection and healthy persons. The experimental group was divided into four groups: SLE group with pulmonary fungal infection. The number of CD3-CD56 NK cells in peripheral blood of patients with pulmonary fungal infection and control group were detected by flow cytometry. Expression of NK receptor NKp44 and TLR 2 and intracellular granzyme B expression. Results: the number of NK cells in peripheral blood of SLE and SLE patients with pulmonary fungal infection was significantly lower than that of the other two groups (P 0.05). There was no significant difference between the two groups (P 0.05), but the pulmonary fungal infection group was higher than the normal group (P 0.05). The expression of granzyme B in peripheral blood of SLE group was significantly lower than that of healthy control group and pulmonary fungal infection group, but in SLE complicated with pulmonary fungal infection group. The expression of granzyme B was significantly higher than that of the healthy control group (P 0.05), and the expression of granzyme B was significantly higher than that of the SLE group and the pulmonary fungal infection group. There was no significant difference between the pulmonary fungal infection group and the healthy control group. The expression of NKp44 in SLE patients with pulmonary fungal infection was significantly higher than that in the other three groups. The expression of TLR2 in SLE group and SLE with pulmonary fungal infection group was significantly higher than that in healthy control group and pulmonary fungal infection group (P0.05). Compared with SLE group, the expression of P0. 05 in SLE with pulmonary fungal infection group was significantly higher than that in healthy control group. Conclusion: the percentage of NK cells and granzyme B in patients with systemic lupus erythematosus is significantly lower than that in the normal group, and its immune defense function is decreased. The expression of TLR2 in SLE patients with pulmonary fungal infection was lower than that in SLE group, but the expression in pulmonary fungal infection group was higher than that in normal group. The expression of NKp44 in patients with systemic lupus erythematosus complicated with fungal infection was higher than that in healthy group and SLE group, and the function of NK cells was activated with the increase of TLR2 and granzyme B.
