【摘要】：目的:觀察2009年和2015年鄉(xiāng)鎮(zhèn)衛(wèi)生院的競爭力變化,探討其關(guān)鍵影響因素,為鄉(xiāng)鎮(zhèn)衛(wèi)生院服務(wù)發(fā)展定位提供參考。方法:利用2010—2016"中國統(tǒng)計(jì)年鑒"、"中國衛(wèi)生和計(jì)劃生育統(tǒng)計(jì)年鑒"的數(shù)據(jù),測算31個(gè)省份2009年和2015年鄉(xiāng)鎮(zhèn)衛(wèi)生院服務(wù)份額水平及變化,用Pearson相關(guān)系數(shù)反映鄉(xiāng)鎮(zhèn)衛(wèi)生院服務(wù)份額與相關(guān)因素的關(guān)系。結(jié)果:相較于2009年,2015年相關(guān)經(jīng)濟(jì)社會(huì)變量均大幅增長。2009年門診份額與農(nóng)村居民可支配收入正相關(guān)(r=0.39),與人均村醫(yī)數(shù)量負(fù)相關(guān)(r=-0.39),2015年的相關(guān)無統(tǒng)計(jì)學(xué)意義;2009年住院服務(wù)份額與人均政府衛(wèi)生投入和人均醫(yī)院衛(wèi)技人員數(shù)中度負(fù)相關(guān)(r=-0.58、-0.56),2015年則不再相關(guān),與每千居民鄉(xiāng)鎮(zhèn)衛(wèi)生院衛(wèi)技人員數(shù)2009年不相關(guān),2015年呈中度正相關(guān)。結(jié)論:新醫(yī)改以來,鄉(xiāng)鎮(zhèn)衛(wèi)生院門診服務(wù)競爭力略有上升但住院競爭力明顯下降,其服務(wù)競爭力和體系內(nèi)上下游競爭者的關(guān)系以及與經(jīng)濟(jì)因素的關(guān)系均在減弱,轉(zhuǎn)由人力資源和床位水平?jīng)Q定。要充分發(fā)揮鄉(xiāng)鎮(zhèn)衛(wèi)生院的效能,關(guān)鍵是在界定服務(wù)功能基礎(chǔ)上提升資源配置水。
[Abstract]:Objective: to observe the changes of the competitiveness of township health centers in 2009 and 2015 and to explore the key influencing factors so as to provide reference for the orientation of service development of township hospitals. Methods: using the data of China Statistical Yearbook 2010-2016 and China Health and Family Planning Statistical Yearbook, the service share level and changes of township health centers in 31 provinces in 2009 and 2015 were measured. The correlation coefficient of Pearson was used to reflect the relationship between service share and related factors in township health centers. Results: compared with 2009, the relative economic and social variables increased significantly in 2015. In 2009, the proportion of outpatient clinics was positively correlated with the disposable income of rural residents (r = 0.39), and negatively correlated with the number of village doctors per capita (r-0.39). There was no statistical significance for 2015; The share of hospital service in 2009 was negatively correlated with the health input per capita and the number of health workers per hospital (r: 0.58% -0.56), but not with the number of health workers per thousand residents in township health centers in 2015. There was a moderate positive correlation in 2015. Conclusion: since the new medical reform, the outpatient service competitiveness of township health centers has increased slightly, but the inpatient competitiveness has declined obviously, and the relationship between the service competitiveness and the upstream and downstream competitors in the system as well as the economic factors has been weakened. The transfer is determined by the level of human resources and beds. In order to give full play to the effectiveness of township health centers, the key is to improve the allocation of water resources on the basis of defining the service function.
