基于洛侖茲曲線和基尼系數(shù)的四川衛(wèi)生資源配置公平性評(píng)價(jià)

【摘要】:目的:考察四川省衛(wèi)生資源配置公平性,為四川衛(wèi)生資源的優(yōu)化配置提供借鑒。方法:以洛倫茲曲線(Lorenz curve)、基尼系數(shù)(Gini coefficient)對(duì)四川省主要衛(wèi)生資源指標(biāo)(機(jī)構(gòu)、床位、衛(wèi)技人員、醫(yī)生、護(hù)士)在21個(gè)市(州)配置的人口和地域分布進(jìn)行評(píng)價(jià),并對(duì)2008—2012年衛(wèi)生資源配置情況進(jìn)行趨勢(shì)分析。結(jié)果:機(jī)構(gòu)、床位、衛(wèi)技人員、醫(yī)生、護(hù)士人口公平性基尼系數(shù)分別為0.2254,0.1547,0.4714,0.4546,0.5187;地域公平性基尼系數(shù)分別為0.6438,0.7454,0.7567,0.7514,0.7721;2008—2011年,各衛(wèi)生資源指標(biāo)均保持平穩(wěn)或呈下降趨勢(shì),2011年人口和地域分布公平性最好,但2012年衛(wèi)生人力資源指標(biāo)基尼系數(shù)出現(xiàn)反彈,均超過警戒值。結(jié)論:四川衛(wèi)生資源總量實(shí)現(xiàn)快速增長(zhǎng),市(州)之間衛(wèi)生資源配置極不均衡,衛(wèi)生人力資源配置亟待優(yōu)化,建議應(yīng)調(diào)整存量、優(yōu)化增量、合理布局,提高醫(yī)療衛(wèi)生服務(wù)的可及性和有效性。
[Abstract]:Objective: to investigate the equity of health resources allocation in Sichuan Province and to provide reference for the optimal allocation of health resources in Sichuan province. Methods: the population and geographical distribution of the main health resource indicators (institutions, beds, health technicians, doctors and nurses) in 21 cities (counties) in Sichuan Province were evaluated with the Lorenz curve), coefficient. The trend of health resource allocation in 2008-2012 was analyzed. Results: the Gini coefficient of population equity in institutions, beds, health professionals, doctors and nurses was 0.2254 / 0.1547 / 0.4744 / 0.4546 / 0.5187, and the Gini coefficient of geographical equity was 0.64380.0.7454/ 0.75670.7514 / 0.7721 / 2008a, respectively. All health resource indicators remained stable or decreased, and the fairness of population and geographical distribution was the best in 2011, but the Gini coefficient of health human resources index rebounded in 2012, which exceeded the warning value. Conclusion: the total amount of health resources in Sichuan is increasing rapidly, the allocation of health resources among cities (prefectures) is very unbalanced, and the allocation of health human resources needs to be optimized urgently. It is suggested that the stock should be adjusted, the increment should be optimized and the distribution of health resources should be reasonable. Improve accessibility and effectiveness of health services.
