【摘要】:目的:通過對(duì)喀什地區(qū)醫(yī)療機(jī)構(gòu)進(jìn)行系統(tǒng),全面的調(diào)查,獲得衛(wèi)生人力資源基本數(shù)據(jù),分析喀什地區(qū)衛(wèi)生人力資源配置現(xiàn)狀,從地域分布、人口分布等方面評(píng)價(jià)衛(wèi)生人力資源公平性,找出喀什地區(qū)衛(wèi)生人力資源配置中存在的問題,為進(jìn)一步推動(dòng)喀什地區(qū)衛(wèi)生事業(yè)的發(fā)展提供參考依據(jù)。方法:采用問卷調(diào)查法,對(duì)喀什地區(qū)衛(wèi)生機(jī)構(gòu)進(jìn)行問卷調(diào)查,采用流行病學(xué)描述性分析方法對(duì)喀什地區(qū)衛(wèi)生人力資源數(shù)量、分布和結(jié)構(gòu)機(jī)型分析,同時(shí)對(duì)2010-2014年喀什地區(qū)衛(wèi)生機(jī)構(gòu)衛(wèi)生人力資源的動(dòng)態(tài)變化進(jìn)行統(tǒng)計(jì)分析,并與全疆的平均水平進(jìn)行比較;采用洛倫茨(Lorenz)曲線與基尼(Gini)系數(shù)對(duì)喀什地區(qū)衛(wèi)生人力資源地域及人口分布的公平性進(jìn)行評(píng)價(jià)。結(jié)果:截止2014年年底,喀什地區(qū)共有醫(yī)療衛(wèi)生機(jī)構(gòu)3168個(gè),其中綜合性醫(yī)院108家,維吾爾醫(yī)院11家,?漆t(yī)院10家,社區(qū)衛(wèi)生服務(wù)中心17家,鄉(xiāng)鎮(zhèn)衛(wèi)生院163家,村衛(wèi)生室2312家,門診部13家,婦幼保健院,13家,疾病預(yù)防控制中心14家,衛(wèi)生監(jiān)督檢驗(yàn)機(jī)構(gòu)13家,計(jì)劃生育服務(wù)機(jī)構(gòu)13家;喀什地區(qū)衛(wèi)生人力資源總量為20368人,其中衛(wèi)生技術(shù)人員17283人、其他技術(shù)人員487人、管理人員982人、工勤技能人員1616人?κ驳貐^(qū)公立醫(yī)院衛(wèi)生人員共12812人,占62.90%、鄉(xiāng)鎮(zhèn)及以下醫(yī)療機(jī)構(gòu)5707人,占28.02%、公共衛(wèi)生機(jī)構(gòu)1285人,占6.31%、診所、醫(yī)務(wù)室562人,占2.76%;喀什地區(qū)衛(wèi)生人力資源職稱分布主要集中在員/士及以下,員/士及以下人員分別占衛(wèi)生技術(shù)人員、其他人員、管理人員、工勤技能人員的66.13%、52.15%、73.32%、100%?κ驳貐^(qū)衛(wèi)生人力資源學(xué)歷分布主要集中在中專及以下,中專及以下人員分別占衛(wèi)生技術(shù)人員、其他人員、管理人員、工勤技能人員的57.96%、25.05%、57.23%、88.92%。年齡分布主要集中在35-44歲,35-44歲人員分別占衛(wèi)生技術(shù)人員、其他人員、管理人員、工勤技能人員的35.41%、33.06%、31.06%、33.04%;喀什地區(qū)按人口分布的基尼(Gini)系數(shù)由小到大依次為執(zhí)業(yè)醫(yī)師(G=0.4759),注冊(cè)護(hù)士(G=0.4773),衛(wèi)生技術(shù)人員(G=0.4982),按地理面積分布的基尼(Gini)系數(shù)由小到大依次為注冊(cè)護(hù)士(G=0.6211),衛(wèi)生技術(shù)人員(G=0.6321),執(zhí)業(yè)(助理)醫(yī)師(G=0.759)。結(jié)論:喀什地區(qū)衛(wèi)生人力資源總量不足,配置不合理,衛(wèi)生人力資源地域分布不平衡,公平性差,職稱結(jié)構(gòu)、專業(yè)結(jié)構(gòu)分布不合理。建議依據(jù)科學(xué)的理論與方法制定符合實(shí)際的衛(wèi)生人才發(fā)展規(guī)劃,建立科學(xué)的區(qū)域衛(wèi)生配置標(biāo)準(zhǔn)加強(qiáng)培訓(xùn)力度,提高衛(wèi)生人力資源綜合水平。
[Abstract]:Objective: to obtain the basic data of health human resources and analyze the present situation of the allocation of health human resources in Kashi area through the systematic and comprehensive investigation of medical institutions in Kashi area and to analyze the regional distribution of health human resources. To evaluate the fairness of health human resources in terms of population distribution and to find out the problems existing in the allocation of health human resources in Kashi. Methods: questionnaire survey was used to investigate the health institutions in Kashi area. Epidemiological descriptive analysis was used to analyze the quantity, distribution and structure of health human resources in Kashi area. At the same time, the dynamic changes of health human resources in health institutions in Kashi region from 2010 to 2014 were statistically analyzed and compared with the average level of the whole Xinjiang. The Lorenz curve and Gini coefficient were used to evaluate the fairness of the distribution of health human resources in Kashi area. Results: by the end of 2014. There are 3168 medical and health institutions in Kashi, including 108 general hospitals, 11 Uygur hospitals, 10 specialized hospitals, 17 community health service centers and 163 township health centers. 2312 village clinics, 13 outpatient departments, 13 maternal and child health hospitals, 14 disease prevention and control centers, 13 health supervision and inspection institutions and 13 family planning services; The total number of health human resources in Kashi is 20368, including 17283 health technicians, 487 other technicians and 982 managers. The total number of health personnel in public hospitals in Kashi is 12812, accounting for 62.90, and that in township and below medical institutions is 5,707, accounting for 28.02%. In public health institutions, 1285 persons (6.31%) and 562 people (2.76%) in clinics and infirmary; The professional titles of health human resources in Kashi area are mainly distributed in the personnel / personnel and below, who account for 66.13% of the health technical personnel, other personnel, management personnel and workers' skills respectively. 52.15 / 73.320.The distribution of the educational background of health human resources in Kashi area is mainly concentrated in the technical secondary school and below, the technical secondary school and the personnel below account for the health technical personnel and other personnel respectively. The management personnel, the working-hand skilled personnel 57.96 and 25.05 are 57.232.The age distribution is mainly in the 35-44 years old. The 35-44 years old persons accounted for 35.41% of the health technicians, other personnel, management personnel, workers and skilled workers respectively, and 33.060.31.06and 33.04; The Gini coefficient of the population distribution in Kashi area was 0.4759 and 0.4773 respectively. The Gini coefficient of health technicians was 0.4982, and the Gini coefficient of Gini in the distribution of geographical area was from small to large in order of registered nurses' Gou (0.6211). The health technicians were 0.6321, and the practicing (assistant) doctors were 0.759.Conclusion: the total amount of health human resources in Kashi area is insufficient and the allocation is unreasonable. The regional distribution of health human resources is unbalanced, the equity is poor, the structure of professional title and professional structure is unreasonable. It is suggested that the development plan of health talents should be formulated according to scientific theory and method. To establish scientific regional health allocation standards and strengthen training to improve the comprehensive level of health human resources.
