帕金森病患者非運動癥狀調(diào)查及其生活質(zhì)量影響因素分析

【摘要】:目的:研究帕金森病患者的非運動癥狀、生活質(zhì)量及其相關(guān)影響因素分析。 方法:選取2011年9月至2013年8月在南昌大學第二附屬醫(yī)院神經(jīng)內(nèi)科門診就診及住院部住院的原發(fā)性帕金森。≒arkinson disease,PD)患者116例。采用非運動癥狀篩查量表(Non-Motor Symptoms Questionnaire,,NMSQuest)、漢密爾頓抑郁量表(Hamilton Depression Scale,HAMD)、39項帕金森病生存質(zhì)量調(diào)查表(the39-item Parkinson’s Disease Questionnaire,PDQ-39)、改良Hoehn-Yahr分級量表(Hoehn-Yahr stages,H-Y)、帕金森病睡眠量表(Parkinson’s disease sleep scale,PDSS)評估PD患者的非運動癥狀、抑郁程度、生活質(zhì)量以及睡眠質(zhì)量。 結(jié)果:116例PD患者中有113例存在NMS,平均個數(shù)為(8.12±5.4)。便秘發(fā)生率最高,占74.1%;其次為記憶力下降和對周圍發(fā)生的事情失去興趣,分別占57.7%和49.1%。NMS發(fā)生數(shù)與病程(r=0.327)、H-Y分級(r=0.346)、HAMD評分(r=0.356)及PDSS評分(r=-0.300)相關(guān)。多元線性回歸分析顯示病程、H-Y分級、抑郁是NMS發(fā)生數(shù)的主要影響因素。PD患者生存質(zhì)量受損最明顯的是日;顒,其次為認知力,運動能力;交往和社會支持維度受損最輕。相關(guān)分析得出NMS個數(shù)(r=0.392)、H-Y分級(r=0.590)、HAMD評分(r=0.445)、PDSS評分(r=-0.594)與生存質(zhì)量密切相關(guān)。多元線性回歸分析得出睡眠質(zhì)量、H-Y分級和抑郁是影響PD患者生存質(zhì)量的主要因素。 結(jié)論:1、非運動癥狀在PD患者中發(fā)生率高,便秘、記憶力下降、缺乏興趣是最常見的NMS;2、病程、病情程度、抑郁是NMS發(fā)生數(shù)的主要影響因素;3、PD患者抑郁、睡眠障礙發(fā)生率高,最常見抑郁癥狀是興趣缺乏,夜尿是最常見睡眠問題;4、PD患者生存質(zhì)量受損最明顯的是日;顒印⒄J知力、運動能力;5、睡眠質(zhì)量、病情程度和抑郁是影響PD患者生存質(zhì)量的主要因素。
[Abstract]:Objective: to study the non-motor symptoms, quality of life and related factors of Parkinson's disease. Methods: from September 2011 to August 2013, 116 patients with primary Parkinson's disease (Parkinson disease,PD) were enrolled in the Department of Neurology, second affiliated Hospital of Nanchang University. Nonmotor symptom screening scale (Non-Motor Symptoms Questionnaire,NMSQuest), Hamilton Depression scale (Hamilton Depression Scale,HAMD), and 39 items of Parkinson's Disease quality of Life (the39-item Parkinson's Disease Questionnaire,PDQ-39) were used. Modified Hoehn-Yahr rating scale (Hoehn-Yahr stages,H-Y) and Parkinson's Disease Sleep scale (Parkinson's disease sleep scale,PDSS) were used to evaluate non-motor symptoms, degree of depression, quality of life and sleep quality of PD patients. Results: the average number of NMS, in 113 patients with PD was (8.12 鹵5.4). The incidence of constipation was the highest (74.1%). The second was memory loss and loss of interest in the surrounding events, accounting for 57.7%, and the incidence of 49.1%.NMS and course of disease (rn 0.327), H-Y grade (rr 0.346), HAMD score (r = 0.356) and PDSS score (r = -0.300) were correlated. Multiple linear regression analysis showed that the course of disease, H-Y grade and depression were the main factors influencing the occurrence of NMS. The most significant impairment of quality of life in PD patients was daily activities, followed by cognitive ability and motor ability. The dimension of communication and social support is least damaged. Correlation analysis showed that the number of NMS (r = 0.392) and H-Y grade (r = 0.590), HAMD) (r = 0.445), PDSS) were closely related to the quality of life. Multiple linear regression analysis showed that sleep quality, H-Y grade and depression were the main factors affecting the quality of life in patients with PD. Conclusion: 1 the incidence of non-motor symptoms in PD patients is high, constipation, memory decline, lack of interest is the most common course of NMS;2, the degree of illness, depression is the main factors affecting the number of NMS; 3The incidence of depression and sleep disorder in PD patients was high, the most common depressive symptom was lack of interest, the nocturnal urine was the most common sleep problem, and the most obvious impairment of quality of life in PD patients was daily activity, cognitive ability and motor ability. 5. Sleep quality, severity of illness and depression were the main factors affecting the quality of life in patients with PD.
