【摘要】：目的調(diào)查育齡期女性帕金森病(PD)患者受經(jīng)期影響的患病率及臨床特征。方法采用調(diào)查問卷、病例對照研究,連續(xù)選取自2015年6月~2016年1月我院門診及天壇醫(yī)院PD專病門診發(fā)病年齡在50歲以下女性PD患者,記錄受經(jīng)期影響的病例數(shù),分為經(jīng)期相關(guān)組和與經(jīng)期無關(guān)組,分析兩組臨床的異質(zhì)性。同時收集兩組發(fā)病年齡、文化程度、初始癥狀、病程、有無毒物接觸史、家族史、統(tǒng)一PD評定量表第Ⅲ部分(UPDRS-Ⅲ)評分、改良Hoehn-Yahr(H-Y)分級、貝克焦慮量表(BAI)等資料。結(jié)果共收集175例女性PD患者,經(jīng)期受影響者42例,患病率為24%;將經(jīng)期相關(guān)組與經(jīng)期無關(guān)組分別在非經(jīng)期及經(jīng)期期間進(jìn)行比較,非經(jīng)期期間,兩組在發(fā)病年齡、BAI方面差異有統(tǒng)計(jì)學(xué)意義(均P0.05),而在病程、文化程度、有無毒物接觸史、家族史、UPDRS-Ⅲ、改良H-Y分級等方面則差異無統(tǒng)計(jì)學(xué)意義(均P0.05),但在經(jīng)期期間,兩組在發(fā)病年齡、UPDRS-Ⅲ、改良H-Y分級、BAI等方面均存在明顯統(tǒng)計(jì)學(xué)意義(均P0.000)。結(jié)論女性育齡期PD患者易受經(jīng)期影響,臨床癥狀惡化,表現(xiàn)為運(yùn)動癥狀加重,更易合并焦慮,提示該類患者日常生活能力及生活質(zhì)量更差,應(yīng)提醒臨床醫(yī)生的識別。
